A B S T R A C T PurposePrior studies have suggested that melatonin, a frequently used integrative medicine, can attenuate weight loss, anorexia, and fatigue in patients with cancer. These studies were limited by a lack of blinding and absence of placebo controls. The primary purpose of this study was to compare melatonin with placebo for appetite improvement in patients with cancer cachexia. Patients and MethodsWe performed a randomized, double-blind, 28-day trial of melatonin 20 mg versus placebo in patients with advanced lung or GI cancer, appetite scores Ն 4 on a 0 to 10 scale (10 ϭ worst appetite), and history of weight loss Ն 5%. Assessments included weight, symptoms by the Edmonton Symptom Assessment Scale, and quality of life by the Functional Assessment of Anorexia/Cachexia Therapy (FAACT) questionnaire. Differences between groups from baseline to day 28 were analyzed using one-sided, two-sample t tests or Wilcoxon two-sample tests. Interim analysis halfway through the trial had a Lan-DeMets monitoring boundary with an O'Brien-Fleming stopping rule. Decision boundaries were to accept the null hypothesis of futility if the test statistic z Ͻ 0.39 (P Ն .348) and reject the null hypothesis if z Ͼ 2.54 (P Յ .0056). ResultsAfter interim analysis of 48 patients, the study was closed for futility. There were no significant differences between groups for appetite (P ϭ .78) or other symptoms, weight (P ϭ .17), FAACT score (P ϭ .95), toxicity, or survival from baseline to day 28. ConclusionIn cachectic patients with advanced cancer, oral melatonin 20 mg at night did not improve appetite, weight, or quality of life compared with placebo.

(20) • MALIGNANT TUMOURSАннотация В 21-м веке достигнуты значительные успехи в лекарственном лечении злокачественных опухолей. Применение новых препаратов позволяет увеличить время до прогрессирования заболевания, продолжительность жизни больных, улучшить ее качество. В последнее время активно изучается противоопухолевая активность гормона эпифиза мелатонина. Этой теме посвящены многочисленные опыты in vitro/ in vivo, в которых показана способность мелатонина тормозить злокачественный рост и потенцировать действие цитостатиков. В данном обзоре освещены данные о использовании мелатонина при лечении пациентов с солидными опухолями. Мелатонин использовался как в монотерапии, так и в комбинации с химиотерапией и иммунотерапией. Согласно результатам этих исследований применение мелатонина приводит к увеличению объективных ответов и общей выживаемости пациентов и к снижению частоты побочных эффектов химиотерапии. Однако этот гормон эпифиза до сих пор рутинно не используется в клинической онкологии и его активность следует изучать в дальнейшем в рамках рандомизированных клинических исследований. AbstractNowadays great successes have been reached in chemotherapy treatment of cancer. New medications let increase time to progression, overall survival and to improve a quality of life. Recently, antitumor activity of melatonin is actively investigated. Many in vitro/in vivo studies are devoted to this topic. In these trials was shown ability of melatonin to inhibit tumor growth and to potentiate the cytotoxic effects of chemotherapy. This review describes the use of melatonin in the treatment of patients with solid tumors. Melatonin has been used in both monotherapy and in combination with chemotherapy and immunotherapy. According to these studies, the use of melatonin increases the objective response and overall survival of patients and reduce the incidence of side effects of chemotherapy. However, this epiphyseal hormone does not routinely used in clinical oncology and will be must to investigate it in more clinical trials.
