Introduction: One of the most common neurologic disease in Acquired Immunodeficiency Syndrome (AIDS) caused by Human Immunodeficiency Virus (HIV) is represented by progressive multifocal leukoencephalopathy (PML), being caused by John Cunningham (JC) polyoma virus. Case presentation: We report a case of a 27 years old women, HIV-positive since childhood, under specific antiretroviral therapy with good adherence to it in that period but starting with adolescence adherence to highly active antiretroviral therapy (HAART) decreased. In this context her HIV viral load increased to a 690.000 copies/ml, and CD4 collapsed at 57 cells/mmc. She presented in our clinic with ataxic left hemiparesis, truncal ataxia and left hemi-hypoesthesia. Cerebrospinal fluid (CSF) showed a slightly pleocytosis and polymerase chain reaction performed from CSF diagnosed John Cunningham (JC) virus. Once diagnosis established, we reinitiated HAART, but some neurologic disorders persisted like difficulty of the left upper member, having the modified Rankin scale (mRS) of 5. The patient started a multidisciplinary rehabilitation (MDR) treatment, specifically adapted. It consisted of 4 sessions of neuromotor treatments, 20 sessions of massages, and 12 sessions of occupational therapy. According to MDR, the patient improved the ataxic walking, without support, presenting an improvement mRS of 3. Conclusions: AIDS patients with PML could require a prolonged MDR treatment for neurological disorders and rehabilitation treatment promptly should be applied when such diagnosis is suspected. Knee osteoarthritis (OA) represents one of the most frequently treated pathology within the rehabilitation departments all over the world; it involves people over 40, mostly overweight subjects. Our present study, performed on 65 patients diagnosed with knee OA, treated with physiotherapy, hydrotherapy and peloidotherapy in the Balneal Sanatorium of Techirghiol, Romania, tries to demonstrate the importance of the natural biotherapeutic factors on the pain and knee functioning in this pathologic condition. All the patients were assessed using the Visual Analogue Scale (VAS) and Functional Independence Measures (FIM), prior and after the treatment. Exclusion criteria were the major pathologic conditions that represent contraindications for physical therapies, and associated neurologic and post-traumatic conditions, also. The results demonstrate the reduction, with almost 50%, of the initial pain intensity score and the improvement of general functioning with almost 35%. So, we conclude that natural therapeutic factors can represent a safe nonpharmacological therapeutic way for improving the functionality of these patients, directly related with pain intensity, with low cost and high accessibility. RezumatOsteoartrita genunchiului (OA) reprezintă una dintre cele mai frecvent tratate patologii din cadrul departamentelor de reabilitare din întreaga lume, care implică persoane de peste 40 de ani, în principal subiecți supraponderali. Prezentul studiu, efectuat pe 65 de pacienți diagnosticați cu OA la genunchi, tratați cu fizioterapie, hidroterapie și peloidoterapie în Sanatoriul Balnear și de Recuperare din Techirghiol, România, încearcă să demonstreze importanța factorilor bioterapeutici naturali asupra durerii și funcționării genunchiului în această stare patologică. Toți pacienții au fost evaluați utilizând scala analogică vizuală (VAS) și măsurarea independenței funcționale (FIM), înainte și după tratament. Criteriile de excludere au fost principalele condiții patologice care reprezintă contraindicații pentru terapiile fizice, precum și condițiile neurologice și posttraumatice asociate. Rezultatele demonstrează reducerea, cu aproape 50%, a scorului inițial al intensității durerii și îmbunătățirea funcționării generale cu aproape 35%. Deci, concluzionăm că factorii terapeutici naturali pot reprezenta o modalitate terapeutică sigură non-farmacologică de îmbunătățire a simptomatologiei acestor pacienți, direct legată de intensitatea durerii, cu costuri reduse și accesibilitate ridicată.

Introduction. Disc herniation means the movement of an intervertebral disc. Lumbar disc herniation is an evolutionary phase of lumbar vertebral discopathy , by de Seze classification: phase I (low back pain), phase II (lumbar pain and paravertebral contracture), phase III (discal hernia, with three stages , radicular pain, paresthesias and motor deficiency). Materials and Methods. We are presenting the situation of a female patient, aged 62, from the urban area, with confirmed vices of smoking and sedentary lifestyle, which presented in Emergency Room in Constanta for lumbosciatalgias and paresthesias, impaired walking and presence of antalgic positions that required hospitalization in Neurosurgery section. The patient has a 15 points Glasgow score, with medical history of Hypertension, Rheumatoid Arthritis with corticotherapy, minor stroke and lymph node Tuberculosis. The muscular and osteoarticular system: apparently integral, with difficulty for active movements. The magnetic rezonance examination reveals 2 lumbar disc hernias at level L4. The neurosurgical treatment was applied: discectomy in the L4 disc herniation bilaterally with the removal of the disc fragments. Subsequently, the patient was transferred to the medical rehabilitation department from Techirghiol Sanatorium. The patient was evaluated clinically, functionally in dynamics to track the effectiveness of the neuromotor rehabilitation program. Results. Through the program of early rehabilitation established, the therapeutic yield was significant with the improvement of the clinical symptomatology as well as the marked increase of the functional parameters, assuring the patient a high degree of mobility, of autonomy, but also of reintegration in the social and family life. Conclusions. The peculiarity of this case was the critical condition of the patient at the hospitalization, corticodependence, which makes the surgery but also the medical rehabilitation much complicated by functional osteoporosis and low bone consistency. Neurosurgery together with medical rehabilitation have sounded excellent together with in many cases, with ability to restore strength, functionality as well as better health of patients, which creates an indispensability between the two specializations.
