Bilobed Flap for Reconstruction of a Large Keratoacanthoma of the Thumb

Abstract: We present the bilobed flap principle which can be used to reconstruct of large defects located on the dorsum of the thumb. The flap provides similar texture, colour and thickness as adjacent hand skin. The final results were excellent both functionally and cosmetically.

“…2,3,7 It is a versatile skin flap, increasingly used in other anatomical locations. 1,[4][5][6] This clinical case demonstrates that the bilobed flap is a good option for single stage closure of larger defects of the neck avoiding the need of tissue expanders, free tissue transfers or skin grafts. In addition to the simplicity of the procedure, good aesthetic and functional results are achieved.…”

“…Primary closure of the defect is not always feasible and skin grafting or local flaps are thus necessary. 1,2 The bilobed flap is a double transposition flap that is designed to recruit adjacent skin and soft tissue from areas of laxity to areas of skin deficiency. It is commonly used in small-to-medium size skin defects of the face, particularly of the nose, where skin is less mobile.…”

“…Few reports have described their use after traumatic finger injuries, on the proximal thumb after Mohs micrographic surgery, or after excision of digital mucus cysts on the distal phalanx. [2][3][4] The Procedure…”

“…It is used extensively in the reconstruction of small-to-moderate-sized cutaneous facial defects [ 6 ] and, more recently, in lower limb defects [ 7 , 8 ]. In the hand, it has been used for coverage of small defects such as those resulting from digital mucous cyst excision [ 9 ], for the reconstruction of dorsal thumb defect following tumor excision [ 10 ], and for reconstruction of the webspace in syndactyly release [ 11 ]. In this case report, we present the use of a bilobed flap as a useful option for coverage of a dorsal finger defect over the proximal phalanx.…”
