Biliary dilatation and duodenal intussusception secondary to enteric duplication cyst: MDCT diagnosis

Abstract: Enteric duplication cysts are rare congenital anomalies that may occur anywhere along the gastrointestinal tract, most commonly involving the small bowel. The distal ileum, jejunum, and duodenum are affected in descending order of frequency. We describe a case of biliary dilatation and duodenal intussusception caused by an enteric duplication cyst in an adult patient. To our knowledge, there are no other reported cases of this entity in an adult in the English literature. Multidetector computed tomography (MDC… Show more

“…The most common presenting symptom is abdominal pain; the most common associated finding is pancreatitis. Intussusception of a duodenal duplication cyst is very rare, of which there have only been two other reports in the literature [3,4]. Other, more commonly described lead points causing duodenal intussusceptions, described in the literature include duodenal adenomas, lipomas, hamartomatous polyps, and hyperplasia [5].…”

“…The most common CT manifestation was an intraluminal mass, seen in 90% (27/ 30) of cases; however, this sign is not specific for intussusception. Several characteristic CT findings were reported, including a target-like mass, a sausage-shaped mass, or a reniform (pseudokidney) mass [19,27,28]. It appears as a target-like mass when the beam is perpendicular to the longitudinal axis but as a sausageshaped or a reniform mass when the CT beam is parallel to its longitudinal axis.…”

“…In general, most cases (90%) were revealed to be tumor-related and benign tumors do so more frequently because they are more common and mobile. In addition, five (10%) patients were secondary to a non-neoplastic lesion, including one duodenal diverticulum [7] and four duodenal duplication cysts [16,27,36,43]. The incidence in each part of the duodenum was dependent on the location of these lesions ( Table 1).…”

“…Jo et al [7] defined 3 criteria for the diagnosis, including the presence of an intimate attachment to the native gastrointestinal tract, a smooth muscle coat, and an alimentary mucosal lining. Digestive duplication cyst occurs most commonly in the distal ileum, followed by the esophagus and duodenum [7,12].…”
