Bile duct injuries during open and laparoscopic cholecystectomy in the laparoscopic era: alarming trends

Abstract: In the laparoscopic era, OC is associated with a high number of BDIs, if minor BDIs are included. Excluding some major LC BDIs, BDIs are, as a rule, missed at the time of surgery. More than 90% of Amsterdam types A, B, and C BDIs can be treated endoscopically, whereas type D BDI remains an absolute indication for surgery.

“…There are also results on the opposite trend – BDI injuries having become more common after OC – which raises concern on surgeons in training not learning appropriate technique for OC in the laparoscopic era [27]. Patients chosen for conversion or direct OC are however often suffering from a more severe inflammation that makes the tissues more prone to rupture and hence patients who have their surgery finished as an open procedure might be at risk for BDI due to inflamed, rupture-prone tissues rather than inadequate surgical technique.…”

“…In addition, most of the open procedures were performed in the first years of the study period. The reported frequency of bile duct injuries varies considerably in published studies (3,4,15) . Retrospective studies might contain selection bias, thereby giving a too low incidence of injuries.…”

“…It is generally believed that the extra-hepatic bile duct with smaller diameter is more prone to be injured during cholecystectomy [18,19]. For these patients, the success rate of HJ is relatively lower if the proximal bile duct segments of the stenosis are not dilated, circumstances that are present in more than half of patients.…”
