Bile Composition in Patients with Chronic Renal Insufficiency

Abstract: Very little is known about bile composition in the end stage of chronic renal sufficiency. Patients with this condition are either assigned to a dialysis-transplantation programme, or are treated temporarily with a low-protein diet. Our study was designed to determine bile composition both in a group of ten patients treated with a low-protein diet over a long period of time, and in 11 patients on regular haemodialysis. The patients on haemodialysis were found to have increased bile cholesterol and an increased… Show more

“…One study in peritoneal dialysis patients showed that the prevalence of female gender, parathyroid hormone (PTH) levels, and serum creatinine levels was increased in patients having GBS, too (9). However, other studies did not confirm these results and could not find a correlation between PTH levels and GBS (2)(3)(4)(5)8). In the present study, we could not find a relationship between PTH levels, Ca, P, and Ca x P product and GBS either.…”

“…Another small study on 11 regular HD patients showed increased bile cholesterol and increased saturation index, a decrease in primary, and an increase in secondary bile acids in the bile, which mimics the change in the spectrum of bile characteristics for cholecystolithiasis (8). There is no detailed information about the types of GBS in HD patients.…”

“…In man, literary data on the subject are scarce. Mareckova et al [4] in a study of patients with chronic renal failure maintained on a low-protein diet documented similar changes in bile composition to those found in animals. These changes were not detected in haemodialyzed (HD) patients on a normal protein intake.…”

“…The main factors in the pathogenesis of cholesterol GS are: (1) supersaturation of bile with cholesterol; (2) destabilization of bile, a function of the nature of bile acids of the micellar form, and (3) stasis of bile in the gallbladder [11]. Epidemiological studies have shown an increased prevalence of cholesterol GS to be associated with increasing age, female gender, parity, obesity, an inherited predisposition, diabetes mellitus, ileal disease, prolonged total parenteral nutrition, oral contraceptives, and low-protein diet [4]. The last mentioned is of particular relevance in patients with advanced chronic renal insufficiency.…”
