Bilateral total knee arthroplasty after spontaneous osseous ankylosis in rheumatoid arthritis

Abstract: Total knee arthroplasty (TKA) was carried out on both knee joints for spontaneous bony ankylosis due to rheumatoid arthritis (RA). Preoperative fixation angles were 40 degrees . First, the peroneal nerve was released prior to TKA. Quadriceps snip was performed to evert the patella laterally. Bilateral TKAs were carried out using a stabilized prosthesis. The results showed full extension to 70 degrees flexion at 3 years after the surgery. Absence of pain, maintenance of stability, and walking ability were achie… Show more

“…With the ability to take down fusions of long standing duration as a viable alternative, the importance of saving as much bone as possible , especially patella bone stock in cases of knee fusion cannot be overemphasized. In fused knee cases that have good bone stock, owing to capsular contracture in medial and lateral, enough stability for the use of usual prostheses can be achieved, but higher constrain prostheses should always be available (10). …”

“…Hence, it is critical to achieve correction of deformity, equalize the medial and lateral soft tissue tension, and implant the components accurately. Appropriate soft tissue balancing in the form of ligament and capsular release at the time of arthroplasty is essential to the success of the procedure [ 21 , 22 ]. As to the remaining some flexion in operation, it was especially important to properly position the individual components and the resulting overall alignment of the lower extremity in RA knee with one-stage TKA [ 18 ].…”

“…Spontaneous osseous ankylosis can occur in weight‐bearing joints as a rare late stage manifestation of rheumatoid arthritis (Ojima et al ., ); however, the most common predilectional sites affected by the disease are the small joints of the hands and feet. Bilateral marginal erosions of the proximal interphalangeal joints and also of the metacarpo‐phalangeal and metatarso‐phalangeal joints are visible in most affected patients; moreover, osteoporosis is also noted around the affected joints (Waldron, ).…”
