Bilateral Severe Corneal Ulcer in a Patient with Lung Adenocarcinoma Treated with Gefitinib

Abstract: We describe the case of Gefitinib-related bilateral corneal perforation. An 86-year-old female patient had bilateral painless and progressive vision loss due to neurotrophic corneal ulcer, following a 2-month treatment with Gefitinib, a selective epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor for metastatic adenocarcinoma of the lung with confirmed EGFR gene mutation. She had no signs of ocular infection, inflammation, or lid problems to account for the development of corneal damage. Neurotr… Show more

“…Epithelial growth factor, ligand of the EGFR, is secreted from the lacrimal glands and found in high concentration in tears [10]. It has an important role in ocular surface homeostasis such as promoting migration and proliferation of epithelial cells [1]. The EGFR is expressed on the basal cell layer of human corneal, limbal, and conjunctival epithelium, and also in epithelial cells of skin, glands, and hair follicles.…”

“…Epidermal growth factor receptor (EGFR) is a molecule expressed in many tissues, including the ocular surface and periocular adnexal structures. Its main physiological function is to regulate epithelial tissue development and homeostasis [1]. In recent years, EGFR inhibitors (EGFR-Is) have become an important treatment for many malignant solid tumor patients, especially in non-small cell lung cancers (NSCLC) [2].…”

“…The effects of the different therapies of cerebral tumors such as surgery ( 19 – 22 ) radiotherapy, ( 23 ) and systemic chemotherapy ( 24 , 25 ) may also alter the nerve fibers or induce limbal stem cell deficiency ( 26 , 27 ) resulting clinically in NK.…”

“…There are previous case reports in which patients of higher age groups presented with corneal ulceration and thinning within three months of gefitinib therapy [ 6 , 7 ]. This case differs in that a young female on gefitinib for two years presented with only mild ocular symptoms.…”
