Social media provide a unique method of analyzing outcomes and quality in medicine. The purpose of this observational study was to investigate the nature of social media content related to shoulder and elbow (S&E) surgery posted by patients, surgeons, and hospitals. A public search of Instagram for a two-year period yielded 1,177 patient-related posts. A categorical system assessed the perspective, timing, tone, and content of each post. Twitter accounts of 77 S&E specialists from the top five ranked U.S. News & World Report institutions were analyzed for activity and content. 5,246 Twitter and Instagram posts for the institutions were analyzed for frequency and content. Most patient-related posts were by patients (68%), postoperative (82%), positive (87%), and centered on return-to-play for Tommy John (34%), surgical site for shoulder arthroplasty (52%), and activities of daily living for rotator cuff repair (22%). 37% of surgeons had active accounts averaging 46 posts, 87% of which were practice advertisements. Hospitals averaged 273 posts over the 2-year period, focusing on education (38%) and community (18%). S&E patients share outcomes on social media in a positive tone with procedure-dependent emphases. Surgeons on social media use sites for practice augmentation. Hospitals often focused posts towards educating the community.

A man in his mid-80s presented with bilateral posterior fracture dislocations of the humerus after suffering a seizure. He had Parskinson’s disease and lived with his wife at home. His left shoulder was not felt to be reconstructable. The initial treatment plan was to perform reverse total shoulder arthroplasty (rTSA) on the left and non-operatively reduce his right shoulder. A left rTSA was performed, but his right shoulder was unstable due to a glenoid fracture and soft tissue instability. In order to preserve the patient’s quality of life, a right rTSA was performed 4 days later. In the follow-up period, the patient was able to regain enough pain-free range of motion on activities of daily living. The patient died from complications of Parkinson’s disease 10 months postoperatively.

Introduction:Studies that report outcomes after staged bilateral shoulder arthroplasty (BSA) are limited. This study compared the overall improvement between the first and second operated shoulders after BSA.Methods:Preoperative and postoperative motion and patient-reported outcomes for function, pain, and general health were assessed for patients who underwent BSA with a 2-year follow-up. Ultimate outcomes and the efficacy of treatment were compared between the first and second operated shoulders.Results:Seventy-three patients met the inclusion criteria (mean follow-up, 51.4 months). There were no notable differences between the first and second operated shoulders for all preoperative and postoperative variables except for the preoperative 12-item Short-Form Physical Component Score, which was greater in the second shoulder (P = 0.005). The efficacy of treatment was not markedly different except for the Physical Component Score (P = 0.001) and forward elevation (P = 0.01), which were greater after the first surgery.Discussion:Improvements in function, pain, and motion were not markedly different between the first and second shoulder arthroplasty surgeries.
