Bilateral Pathological Hip Fractures in a Patient with a Bipulmonary Transplant: A Case Report

Abstract: The use of immunosuppressive agents to prevent tissue rejection may predispose patients to osteoporosis and fractures. We report a case of bilateral pathological hip fractures in a woman with osteoporosis who had undergone a bipulmonary transplant. To reduce the risk of pathological fractures, patients are given prophylaxes for osteoporosis prior to organ transplantation, but they remain at increased risk of fractures. They should be informed that this side-effect may occur after transplantation.

“…This patient population includes the largest group of lung transplant patients (121 patients) compared to other studies in literature [6,7] and the only study to comment on pancreas transplants undergoing TJA. Our findings demonstrate that TJA patients who have undergone organ transplantation have greater in-hospital complications compared to TJA patients without transplants.…”

“…Such subclinical deficiency will commonly progress silently, without obvious clinical manifestations, until it is suddenly revealed when a spontaneous fracture occurs. The literature suggests that patients usually complain of bilateral groin or thigh pain that persists over a period of a few weeks to several months [[23], [24], [25], [26], [27], [28], [29]], so that these fractures develop gradually and diagnosis is typically delayed. In cases such as this, even if the initial radiographs are negative, the possibility of bilateral hip fracture should be considered, particularly if the pain worsens suddenly or the patient exhibits signs of bone insufficiency [26].…”
