Bilateral Optic Papillitis Following Mycoplasma pneumoniae Pneumonia

Abstract: Mycoplasma pneumoniae is an atypical bacterium that can cause a great variety of respiratory infections and be responsible for ocular involvement such as conjunctivitis, anterior uveitis and very rarely optic neuropathy. We report herein an additional case of bilateral optic disc swelling with profound visual loss following Mycoplasma pneumoniae pneumonia and review the world literature on the ocular manifestations associated with this pathogen.

“…Acute rhabdomyolysis was recently reported in association with M. pneumoniae infection in a 15-year-old patient (41). Ocular manifestations have been reported in children occasionally and include conjunctivitis, anterior uveitis, optic neuropathy, retinitis and retinal hemorrhages, iritis, and optic disk swelling, with or without permanent degradation of vision (289,362). M. pneumoniae has been isolated from the urogenital tracts of males and females and has been cultured from a tubo-ovarian abscess (165).…”

“…However, in the literature various complications are described in the skin (Stevens-Johnson syndrome) [87], the central nervous system [81,123], kidneys [111,129], heart [23, 97], muscles [10] and the eyes [98]. The pathophysiology of extrapulmonary manifestations of MP is not understood.…”

“…Ocular manifestations other than conjunctivitis are uncommon. However, papilloedema [10] and bilateral ON have been described following an acute infection by Mycoplasma pneumoniae [1,7]. It is also well known that Mycoplasma pneumoniae infection is related to about 5 % of cases of GBS [4].…”
