Bilateral Ocular Decompression Retinopathy after Ahmed Valve Implantation for Uveitic Glaucoma

Abstract: Case Report: We report the case of a 29-year-old man who underwent Ahmed valve implantation in both eyes as treatment for uveitic glaucoma, subsequently presenting with bilateral ocular decompression retinopathy in the postoperative period. Discussion: Ocular decompression retinopathy is a rare complication of filtering surgery in patients with glaucoma; however, the course is benign in most cases, with spontaneous resolution of bleedings and improvement of visual acuity.

“…2 The development of ODR commonly follows glaucoma filtering surgery but can also occur after anterior chamber paracentesis, laser iridotomy, or medical treatment. 3 In about 80% of cases, visual acuity is intact or merely reduced and the haemorrhages resolve spontaneously over time with good visual outcomes. 1 Patients with severe visual loss are referred for surgical procedures such as trans-pars plana vitrectomy for vitreous and subhyaloid haemorrhage, with good outcomes.…”

“…[ 1 ] It is frequently associated with glaucoma-filtering surgery but also found in some of the other ocular surgeries such as vitrectomy and even in patients with IOP-lowering medication. [ 2 ] One of the possible underlying mechanisms could be the sudden increase in retinal intravascular flow as a result of hypotony and defective autoregulation of blood vessels. [ 3 ]…”
