Bilateral nerve sparing robotic-assisted radical prostatectomy is associated with faster continence recovery but not with erectile function recovery compared with retropubic open prostatectomy: The need for accurate selection of patients

Abstract: Abstract. Robotic-assisted radical prostatectomy (RARP) shows measurable advantages, compared to conventional open surgery, even if some aspects are, still, under debate. The aim of this study was to compare the potency recovery rate of patients with clinically localised prostate cancer treated by bilateral nerve-sparing (BNS) RARP or retropubic radical prostatectomy (RRP), and secondarily, the urinary continence recovery evaluation and the oncological efficacy. All patients treated with BNS-RARP or BNS-RRP fo… Show more

“…Cancer‐related and surgical factors also contribute to continence outcomes. These factors include clinical T stage, bladder neck preservation, cavernous nerve sparing, preservation of urethral length, and reconstruction of the posterior urethral wall . In the present study, nerve preservation was not a significant factor associated with incontinence.…”

“…There were 4 RCT [25, 26, 28, 77] and 26 prospective [1620, 32, 3437, 3943, 4547, 49, 68, 69, 71, 9496] studies. The result for one study [26] was not computable.…”

“…Overall complications were compared in nine RCT [25, 26, 2830, 76, 106108] and 28 prospective [16, 17, 19, 20, 32, 3437, 39, 41, 43, 4547, 49, 6870, 9496, 109113] studies. From these studies, the overall complication rate was 11.6 % (515/4453) in the robotic arm compared with 21.4 % (693/3245) in the open arm.…”

“…In particular, investigating host‐related factors is important, as it can easily be assessed preoperatively, and surgeons can provide information to patients before surgery. Host‐related and surgical factors associated with recovery of continence include younger age, lower BMI, smaller prostate volume, lower preoperative IPSS, length of membranous urethra and nerve preservation . Regarding surgical types, there seems to be no significant difference in continence outcome between the pure laparoscopic and robotic approach in a systematic review carried out by Ficarra et al .…”
