Bilateral Cutaneous Diffuse Large B-Cell Lymphoma on FDG PET/CT

Abstract: Primary cutaneous diffuse large B-cell lymphoma is an uncommon type of non-Hodgkin lymphomas. A 51-year-old man had 2 gradually enlarging reddish, firm, and painless cutaneous-subcutaneous masses surrounded with some erythematous patches on his back for 3 years. Skin biopsy result was consistent with diffuse large B-cell lymphoma. FDG PET/CT was performed for staging and demonstrated bilaterally increased FDG uptake in the cutaneous masses with hypermetabolic left axillary lymph nodes. After chemotherapy, FDG … Show more

“…Only a few studies and case reports evaluate the potential role of FDG PET/CT in diagnosing primary cutaneous lymphomas [ 52 57 ]. Several studies indicate that FDG PET/CT is more accurate than CT in detecting both cutaneous and extra-cutaneous diseases of T cell lymphomas including mycosis fungoides [ 52 54 ]. We have experienced a total of 8 patients with primary cutaneous lymphomas (including 2 patients of mycosis fungoides), all of which showed FDG-avid cutaneous lesions (Fig.…”

“…In general, DLBCL patients with primary extranodal disease tend to be older, have lower tumour burden and are more likely to be Ann Arbor Stage I 4. One rare extranodal presentation is primary cutaneous DLBCL, which typically manifests with papules, nodules or cutaneous tumours that have discolouration, erythema, plaques and/or ulceration 5 6. Herein we present a rare case of DLBCL presenting as an isolated subcutaneous mass without any visible skin changes and with no nodal involvement.…”

“…Chemotherapy was recommended for this patient because surgery and radiotherapy were not appropriate due to the widespread skin involvement. Extensive, high FDG uptake in the skin should also be differentiated from lymphoma, 10,11 sarcoidosis, 12,13 inflammation, 14 Sweet syndrome, 15 and Grover disease. 16 Our case suggests that cutaneous angiosarcoma can present with diffuse skin involvement, and FDG PET/CT plays an important role in determining the treatment plan to avoid unnecessary surgery and radiotherapy.…”
