Bilateral Compressive Optic Neuropathy Secondary to Bilateral Sphenoethmoidal Mucoceles

Abstract: We have presented a rare case of bilateral posterior sphenoethmoidal sinus mucoceles with bilateral compressive optic neuropathy. While the duration of compression was variably present over a 10-month period, there were nevertheless significant improvements in visual acuity of the right eye and visual fields bilaterally following extensive optic nerve decompression.

“…Reported clinical manifestations include headache (typically retro-orbital), facial pain, anosmia, visual loss due to optic nerve involvement (sudden or gradual, binocular or monocular), monocular or binocular, third, fourth, sixth nerve palsies (in isolation or in combination), and rarely hypopituitarism. [5][6][7][8][9][10][11] Visual loss is more common, in contrast to fronto-ethmoidal mucoceles.…”

“…Compressive optic neuropathy may be caused by ethmoid and/or sphenoid sinus mucoceles or mucopyoceles and/or associated edema and thickening of the sinus walls [7]. Polyps involving the mucosa of the sphenoid sinus also cause compression of the nerve [4].…”
