Bifocal extra- and intradural melanocytoma of the spine: case report and literature review

Abstract: Intradural melanocytoma, although exceedingly rare, requires a thorough work-up to exclude malignant melanoma. With only two previous reports of multifocal melanocytoma published in the literature, standard therapy has not yet been established and complete surgical removal remains the modality of choice. Patients should be closely monitored to detect local recurrence or malignant degeneration. EBR may be considered in cases where total excision is not achievable and reduces risk of local recurrences.

“…A total of five cases have been described in the combination of a posterior fossa mass with a spinal lesion or multiple spine lesions. [ 1 6 13 18 20 ] The resected lesions were all benign on pathology except in the case described by Foit et al ., where one of the two lesions had an intermediate grade [ Table 1 ].…”

“…Case reports of melanocytomas have sometimes also commented on the presence of diffuse leptomeningeal hyperpigmentation of the surrounding dura, as seen in our case. [ 1 5 6 7 ] These are not considered to be the cases of melanocytosis, which usually occurs with underlying cutaneous melanosis. Of all the case reports describing multifocal disease, Ali et al .…”

“…[ 2 ] The majority of spinal melanocytomas remain benign and are intradural extramedullary in location with a predilection for the cervical spine due to the increased concentration of melanocytic cells. [ 3 ] This case stands distinct in highlighting the rare occurrence of a lumbar spinal nerve root extradural melanocytoma, emphasizing the remote possibility of its histological progression into malignant melanoma, tackled with the help of adjuvant radiotherapy (RT) and immunotherapy.…”
