Abstract:The purpose of this study was to evaluate the results of the Universal Total Wrist prosthesis.Seventeen wrist arthroplasties were performed in 15 patients using the Universal Total Wrist prosthesis. The 11 women and 4 men in the study group ranged in age from 45 to 86 years. Fourteen patients had rheumatoid arthritis and one had osteoarthritis. Follow up ranged from 20 to 74 months. Patients were evaluated for range of motion, with the Disabilities of the Arm, Shoulder, and Hand (DASH) survey and radiographically.All range of motion values improved after surgery. Average postoperative motion was 29° dorsiflexion, 38° volar flexion, 7° radial deviation and 17° ulnar deviation. The DASH scores improved with 29 %. Pain score improved in all 15 patients. One prosthesis had to be removed due to component loosening of the carpal plate. The other cases showed no radiographic signs of loosening. One patient had an early prosthetic dislocation and was treated conservatively with success.The Universal Total Wrist prosthesis provides a promising outcome in patients with rheumatoid arthritis.

purpose. To report outcomes of 21 total wrist arthroplasties (TWA) using the Universal 2 prosthesis. Methods. Five men and 14 women aged 44 to 82 (mean, 62) years underwent 21 total wrist arthroplasties for rheumatoid arthritis (n=19) and post-traumatic arthritis (n=2) by a single surgeon using the Universal 2 prosthesis. Pre-and post-operative pain and function were assessed by a single surgeon using the Disabilities of the Arm, Shoulder and Hand (DASH) score and the patient-rated wrist evaluation (PRWE) score. Range of motion, stability, dislocation rate, and neurovascular status were also assessed. Radiographs were evaluated for implant alignment and fit, screw positioning, and implant loosening. results. The mean time to assessment of the range of motion was 3.1 (range, 1.8-3.9) years, and the mean time to assessment of the PRWE score was 4.8 (range, 2.1-7.3) years. The range of motion in each direction and the mean DASH and PRWE scores improved significantly following TWA. Two patients Total wrist arthroplasty using the Universal 2 prosthesis 2012;20(3):365-8 had restricted range of motion, which was treated by manipulation under anaesthetic (after 6 months in one and 8 weeks in the other). One patient underwent excision of a palmar bony bridge. One patient endured extensor pollicis longus rupture and underwent tendon transfer after 5 months. Radiographs revealed no evidence of implant loosening, migration, or malalignment. There was no sign of osteonecrosis in the remaining carpals or metacarpals. conclusion. The Universal 2 TWA achieved significant improvement in range of motion and functional outcome of the wrist, with reduced rates of early joint instability, dislocation, and implant loosening, compared to previous implants. Motion analysis of the wrist joints in patients with rheumatoid arthritis

We investigated the characteristics of the wrist joint motion in patients with rheumatoid arthritis (RA), using a biaxial flexible goniometer. Wrist joint range of motion and velocity were measured on the dominant hand in RA patients (n = 22) and normal individuals (n = 5). We investigated flexion-extension (FE) task, radial-ulnar deviation (RUD) task, and functional motion tasks, such as writing letters or unscrewing the lid of a jar. In normal individuals, there was cooperative coupling of FE and RUD during wrist movement, and this coupling motion was essential for normal wrist movements. On the other hand, in RA patients, wrist joint range of motion was restricted at various degrees, with reduced joint motion velocity that was severe on RUD. Functional wrist motion tasks indicated circumductive movement with both FE and RUD in normal individuals, whereas the direction of movement was limited in RA patients, and results revealed failure of cooperative coupling of FE and RUD. Our results indicate that disturbed coupling of FE and RUD results in difficulties in the cooperative movements and have great influence on the daily activities in RA wrist joint.
