Beyond physiotherapy and pharmacological treatment for fibromyalgia syndrome: tailored tACS as a new therapeutic tool

Abstract: Fibromyalgia syndrome (FMS) is a complex pain disorder, characterized by diffuse pain and cognitive disturbances. Abnormal cortical oscillatory activity may be a promising biomarker, encouraging non-invasive neurostimulation techniques as a treatment. We aimed to modulate abnormal slow cortical oscillations by delivering transcranial alternating current stimulation (tACS) and physiotherapy to reduce pain and cognitive symptoms. This was a double-blinded, randomized, crossover trial conducted between February a… Show more

“…It has been relatively rarely explored as a therapeutic tool to treat central nervous system activity-dependent fibromyalgia symptoms. Only a recent study has reported effects so far [ 76 ], thus, evidence for its efficacy is limited at present [ 76 , 77 ]. Other non-invasive electrical brain stimulation techniques, such as transcranial random noise stimulation (tRNS), have also been explored in the context of fibromyalgia, but their effectiveness is unclear.…”

“…FM is a complex, painful disorder that necessitates a multidisciplinary approach for its therapy [1]. The current drugs for treating FM symptoms include many classes of sedatives, analgesics, and antidepressants [36][37][38]. Nevertheless, not all are well tolerated and they do not cover the full range of symptoms associated with FM [39], thus making it necessary to develop new treatments.…”

“…However, gamma-tACS over bilateral prefrontal cortices had no effect on pain intensity in healthy participants. Another recent study investigated whether the normalization of abnormal brain oscillations by delivering personalized tACS reduces pain in patients with fibromyalgia syndrome ( Bernardi et al, 2021 ). Patients showing spectral power corresponding to higher slow frequencies (1–10 Hz) were stimulated with beta-tACS at 30 Hz, while those showing higher fast frequencies (10.5–30 Hz) were stimulated with theta-tACS at 4 Hz.…”
