DiG chronische Form der arterio-ven6sen Fistel entwickelt sich kontinuierlich aus der akuten Form des Krankheitsbildes, das dabei aber einen tiefgreifenden pathophysiologischen Phasenwandel erf/ihrt. Ohne dig Besprechung der ersten Form entbehrt daher das Verst~ndnis der zweiten notwendiger Voraussetzungen. Es sollen daher die pathophysiologischen Besonderheiten und die Symptomatik beider Formen diskutiert werden, wobei es sich bei der grunds~tzlichen Versehiedenheit der Verh/iltnisse als zweckm/iBig erweist, auf die Pathophysiologie tier akuten und der chronischen Fistel getrenn~ einzugehen.A. Pathophysiologie und Symptomatik der akuten arterio-venSsen Fistel Im Vordergrund des klinisehen Bildes steht bei der akuten Fistel bekanntlieh der Kreislau/koUaps versehieden sehwerer Anspr/igung* mit Taehykardie, BlntdruekabfM1, peripherer Vasoconstriction und Cyanose, 9 27 kMtem SchweiB und r6ntgenologiseh naehweisbarer Herzverklelnerung , der in seiner Erscheinung dem peripheren I~reislanfversagen infolge Volumenmangels entspricht und ~olgendermaBen zustande kommt : l~ber die ~raumatiseh entstandene Kommunikation zwisehen Arterie und Vent verblu~et sigh der arterielle Itoehdruekspeieher in die Vene hinein, da ein betri~ehtlicher Anteil des tterzzeitvolumens, dem Weg des geringsten Str6mnngswiderstandes Iolgend, unter Umgehung der Endstromhahn fiber die Fistel direkt in das ven6se Niederdrucksystem abflieSt. Die resultierende Minderung des peripheren Widerstandes kann 40 % 20, abet auch weir mehr 22, 4a und bis zu 65 % s9 betragen. Infolge des Volumenverlustes aus dem arteriellen Druckspeicher ist der zur Aufrechterhaltung eines normMen arteriellen Blutdrueks notwendige Ffillungszustand des arteriellen Systems nicht lgnger gew/ihrleistet.
