A. Einleitung
BLAD zog 1918 a]s erster eine Sch~digung der Gallenwege durch Pankreasfermente in ErwsSeitdem bearbeiteten 18 Untersueher bis in die jfingste Zeit hinein diese Frage experimentell. Aul3erdem erschienen Mitteilungen fiber klinische Beobachtungen vermuteter Fermentsch~den. Arbeiten fiber die anatomisehen, fermentphysiologischen und pathologischen Grundlagen des pankreatobili~ren bzw. des biliopankreatischen Reflux sollten die ttypothese untermauern helfen. Trotzdem steht auch heute noch die vollst~ndige Kl~rung dieses Kapitels der Pathogenese der GaHenwegserkrankungen aus. Der Widerstreit der Meinungen spiege]t sich bereits in der Stellungnahme der Bearbeiter der diesbezfigliehen Kapitel in den neuesten Auflagen der ttandbfieher, Lehrbficher und modernen Monographien zu diesem Fragenkomplex wieder:~Taoli HELLEI~ scheint bei der ,,perforationslosen galligen Peritonitis" die Bedeutung der Pankreasfermentsch~den einwandfrei zu sein. Ubrigens handle es sieh um das einzige Krankheitsbild, von dem wir das behaupten kfnnten.H~ss dagegen bekennt sich unter dem Eindruek der experimentellen Arbeiten ~JORTHS ZU der Auffassung, dal~ der Pankreassaftrfickflul3 von immer grfl]erer Wichtigkeit fiir die Pathologie der Gallenwege zu werden scheine. Nach HEss hat HJORT~ im Tierexperiment den Beweis erbraeht, dal~ eine reine Trypsin-sch~digung der Gallenwege mfglieh ist.B~CK~A~ erwhhnt im Leber-Gallenwegskapitel des Handbuehs der Inneren Medizin Tierexperimente fiber Fermentsch~den und fiber die Auslfsung yon Gallenwegssehaden durch Pankreassaft. Er selbst jedoeh mi~ der Entstehung der ,,abakteriellen Choleeystitis" keine allzu grol~e Bedeutung bei.KATSC~ und G~LZOW zitieren im Pankreasabschnitt des gleichen Handbuches die Arbeiten yon HJo~ und GRIESS~ANN, enthalten sieh aber einer Stellungnahme.Naeh Bi)c~ sprieht gegen eine grunds~tzlich bakterie]le Genese der Cholecystitis der nieht selten negative Ausfall der bakteriologischen Untersuehung des Gallenblaseninhaltes bei akuter Choleeystitis, weshalb noch andere Faktoren bei diesem Krankheitsbild eine Rolle spielen mi~ssen. Bi/~c~ halt die Seh~digung der Gallenwege durch Pankreasfermente ffir mfglich, weitere Untersuchungen zur Kls dieser Frage aber noeli ffir notwendig.
