Benefits of Early Surgical Repair in Fixed Subaortic Stenosis

Abstract: The data suggest that surgical resection of fixed subaortic stenosis before the development of a significant (> 40 mm Hg) outflow tract gradient may prevent recurrence, reoperation and secondary progressive aortic valve disease.

“…Finally, prompt surgery avoids progressive aortic regurgitation which may develop due to persistently turbulent flow patterns in the LVOT that may cause valve damage. 9) Resection of the accessory tissue and removal of membranous or muscular substrata of obstruction is the standard surgical therapy which allowed, in our experience, for essentially complete relief of LVOT obstruction with no evidence of resultant mitral valve dysfunction.…”

“…[3,4] Progressively increasing aortic valve regurgitation due to the presence of stenosis also increases the risk for infective endocarditis. [7,12,13] Therefore, the timing of surgery should be planned in patients with discrete subaortic membrane, considering the gradient in the LVOT and progression of the aortic regurgitation.…”

“…Brauner and colleagues, 9 showed higher recurrence rate with high early postoperative gradients. We did resection of the membrane without myectomy in all the patients with no significant residual gradients immediate post operatively.…”

“…Some authors however suggest early surgical excision of SAM to prevent aortic insufficiency or LVH. 9 In developing countries like Pakistan, the patients of SAM usually present after development of symptoms such as easy fatigability, chest pain or syncope attacks. We present here our early results of resection of the sub aortic membrane combined with septal myectomy at CPEIC Multan.…”
