Bell's palsy: prognostic accuracy of case history, sialometry and taste impairment

Abstract: A prognostic test in cases of Bell's palsy must be accurate and applicable early in the course of the disease. The case history is examined in 239 patients with Bell's palsy. The prognostic value of the case history, taste impairment and sialometry is investigated in a series of untreated patients who were followed up for 12 months. Sialometry is advocated as a reliable progostic indicator in the individual patient if a strong stimulant is used. None of the other examined parameters discrimates between good an… Show more

“…Parasympathetic fibres originating from the superior salivary nucleus carried by the chorda tympani provide the innervation to the submandibular and sublingual glands. Few papers have studied the salivary flow in BP [ 14 , 23 ]. Ekstrand, over 239 patients affected by BP, found the sialometry to be a reliable prognostic examination to identify patients with good or poor facial outcomes at 12 months, when a powerful secretory stimulus is used.…”

“…Aside from assessing general taste dysfunction, the most common clinical use of electrogustometry is to assess taste function as a marker for the prognosis of Bell's and related palsies such as Ramsay Hunt syndrome, 8 , 112 , 113 although its general value in this regards has been questioned. 114 Electrogustometric thresholds have also been useful in assessing adverse effects of tonsillectomy, 8 chemotherapy for cancer, 115 and diabetes and its development, 116 as well as assessing the success of chorda tympani reconstruction surgery. 117 , 118 …”

“…The questionnaire was based on previous findings and consisted of 4 sections: systemic symptoms, sensory changes, auricular symptoms, and eye symptoms. 2,3 The H-B grading scale was used to clinically assess the severity of Bell’s palsy. 7 The H-B grade was reported at initial presentation, 9 weeks after onset, and 6 months after onset.…”

“…1 Several previous studies have reported the preceding symptoms of Bell's palsy. 2,3 To date, however, very few studies have investigated the frequency of prodromal symptoms in Bell's palsy or the relationship between these symptoms and disease severity.…”
