Understanding the Wellbeing Effects of a Community Music Program for People With Disabilities: A Mixed Methods, Person-Centered Study

People with disabilities face inequalities in mental wellbeing, for which social exclusion is a contributing factor. Musical activities offer a promising but complex intervention, making impacts on a population with highly varied characteristics and needs challenging to capture. This paper reports on a mixed methods, person-centered study investigating a community music intervention for such a population. Three groups of adult service users with varied disabilities (either physical, learning, or both), took part in weekly music workshops in different locations. Music staff, housing and resource center staff, as well as participants and members of their families, took part in semi-structured interviews. A quantitative measure administered by service staff was used to rate service users’ social development. Two lay researchers, both individuals with a disability contributed to all aspects of the study. Interviews were analyzed through thematic analysis. Improvements in individuals’ self-expression, confidence, mood, and social skills were consistent with previous findings. Differences in effect between centers included: Group 1, some of whom had previous experience of workshops, showed an improvement in musical skills; Group 2 showed a mixed response, some participated with enthusiasm but others chose art activities over music workshops; Group 3 had lasting positive impact, this group had very limited opportunities for music due to their rural location. Quantitative analysis showed significant increase over all groups in communication, interaction with others, and joint attention. The intervention was beneficial for participants in separate locations in similar ways, but also highlighted that context and prior experience mediated effects in distinct ways. The lay researchers enhanced the qualitative analysis by emphasizing (1) the importance of recognizing participants’ self-expression in non-verbal modes of communication and (2) the importance of having music staff with a disability to provide a positive role model. This paper proposes that mixed methods person-centered research is the most suitable approach to capture and understand the multiple and varied effects of this complex intervention for a diverse group of participants.

Person-centred approaches to practice recognise the global call to humanise healthcare, where people are valued and their preferences and needs are respected (WHO, 2015). Researchers must also embrace person-centred approaches to further inform person-centredness as the foundation of healthcare policy and practice. Healthcare researchers, policymakers, leaders, educators, and multi-disciplinary practitioners are invited to consider how person-centred methodologies enhance the value and authenticity of research. Person-centred research principles to guide researchers are available (Dewing et al., 2021; McCormack et al., 2017), and one approach proposes an overarching research principle of connectivity; the being, doing, and knowing of the researcher throughout the development, implementation, and dissemination of the research study (Jacobs et al., 2017). An exploration of the principles of attentiveness and dialogue, empowerment and participation, critical reflexivity, and loving kindness that inform connectivity (Jacobs et al., 2017; van Leishout & Peelo-Kilroe, 2021) is presented here. No single methodology is associated with person-centred research, and researchers are encouraged to consider how to incorporate person-centred principles in many different contexts. This paper explores the innovative use of person-centred principles to underpin a constructivist grounded theory (CGT) study illuminating the person-centred nursing process of engaging authentically (McCormack & McCance, 2021). Synergies and connections between the person-centred research principles and Charmaz’s CGT approaches are explored, and further considerations are highlighted. We contend that person-centred research principles support CGT research, explicitly building on Charmaz’s relational approaches and enabling a rigorous foundation for engaging in research, where valuing and respect are central.

Person-centred research practice: the user involvement in research of older adults with first-hand experience of reablement

Abstract User involvement is increasingly common in health-care research, and the ideal is user participation and influence during all research stages. Here we describe and reflect on the processes and outcomes associated with advisory group–researcher collaboration from a person-centred perspective. When planning a study in which older adults’ experiences of reablement were investigated, older adults with previous first-hand experience of reablement participated in an advisory group. We found that the fostering of healthful relationships, in which experiential and research knowledge are considered complimentary and equitable, and all members have the power to exercise their unique roles, seems to be a prerequisite for the co-creation of knowledge. Also, practical arrangements and social relationships constitute important details that are crucial to ensuring contribution from older adults with health-related conditions. While such individuals may be unable to participate during all stages of a research project, their involvement on an advisory level during the initial stages can increase study quality and relevance. Input from the advisory group members contributed to the improvement of the language in the study information sheet, improvement of the study design, development and validation of the interview guide, and insight into how the interviews should be conducted. The personal knowledge and expertise of the advisory group members, which emanated from their immediate sensitivity, contributed to the person-centredness in the study.
