BackgroundDepression accompanying chronic pain (CP) is one of the most common comorbid psychiatric disorders. This study aimed to investigate the effectiveness of transcutaneous auricular vagus nerve stimulation (taVNS) combined with electroacupuncture at Baihui (GV20) and Yintang (GV29) acupoints compared with citalopram.MethodsSixty patients with depression and pain comorbidity were enrolled in a prospective 8-week, single-blind, randomized controlled trial. Participants were randomly assigned to receive either taVNS combined with electroacupuncture treatment (taVNS: 8 weeks, 3 sessions per week; electroacupuncture: 8 weeks, twice per day, no drugs) or citalopram treatment (8 weeks, 40 mg/day). The primary outcome was Montgomery–Åsberg Depression Rating Scale (MADRS). The secondary endpoints were evaluated using the McGill Pain Questionnaire (SF-MPQ), self-reported 36-Item Short Form Survey (SF-36), Pittsburgh Sleep Quality Index (PSQI), Hamilton Depression Rating Scale (HAMD) and Hamilton Anxiety Scale (HAMA).ResultsBoth the taVNS combined with electroacupuncture and citalopram groups had significant reductions in depressive and pain symptoms, as indicated by the decrease in MARDS and SF-MPQ scores. Regarding the analgesic effect, the pain intensity score of the SF-MPQ showed a larger reduction with citalopram than with taVNS combined with electroacupuncture at 6 weeks (P = 0.036). The reduction in the BP score of the SF-36 was higher at week 4 (P = 0.000), with no significant difference observed at week 8 (P = 0.1110). This result indicated that the pain intensity can be improved rapidly with citalopram compared with taVNS combined with electroacupuncture. Similarly, the comparison of PSQI scores at 4, 6, and 8 weeks indicates that there was no significant difference between groups, except in the use of sleeping medications. At week 6, higher medication use was found in the citalopram group than in the taVNS combined with electroacupuncture group (P = 0.049).ConclusionIn summary, compared with citalopram, taVNS combined with electroacupuncture produces similar positive effects on depressive and pain symptoms in patients with depression and chronic pain, which last for at least 8 weeks.

Culture-loaded words refer to words with specific cultural connotations that can express an abstract or a specific concept, which may be related to religious beliefs or social customs, but do not exist in other languages and cultures. Therefore, culture-loaded words bring some difficulties to translation work. Huang Di Nei Jing (《黄帝内经》 Huangdi's Internal Classic) is the foundation of traditional Chinese medicine (TCM) theory, and is listed as the first of the four classics of TCM. It contains a large number of culture-loaded words, which embody the ancient Chinese traditional culture. The translation of culture-loaded words is a difficult but crucial point in the translation of Huangdi's Internal Classic and directly relates to the quality of the translation of Huangdi's Internal Classic as a whole. Taking the two English versions of Maoshing Ni and Li Zhaoguo as examples, this work identifies the culture-loaded words appearing in the chapter “Yin Yang Ying Xiang Da Lun” (阴阳应象大论 Comprehensive Discourse on Phenomena Corresponding to the Yin and Yang). This work studies the strategies and translation process of culture-loaded words in Huangdi's Internal Classic, with a view to contribute to the English translation of TCM classics.

This study takes the multisensor monitoring data of tunnel support structure and surrounding rock mechanical properties during TBM construction as the research object, integrates the monitoring data of multisensor through an intelligent optimization algorithm, and explores the self-consistent fusion characteristics between the multisensor structural mechanical properties and the field monitoring data. At the same time, the support structure parameters and construction progress in the tunnel construction process are dynamically adjusted and scientifically integrated through the information feedback of monitoring data. The research results are as follows: (1) The tunnel excavation rate is different under different rock strengths and surrounding rock levels. The overall performance is that the harder the surrounding rock, the lower the surrounding rock level, and the faster the tunnel excavation rate. The relationship between the FPI parameters of surrounding rock and the deformation of tunnel surrounding rock is closer, and the correlation coefficient reaches 98%. The mechanical properties of the surrounding rock have the greatest influence on its deformation. (2) The rotation speed, thrust, and torque of TBM machinery as well as the surrounding rock workability index and the tunnel surrounding rock deformation maintain a good fitting relationship, with a fitting rate of more than 85%. After the improved PSO-BP algorithm is used to calculate and fit the four parameters of torque, rotational speed, thrust, and surrounding rock drivable coefficient during TBM mechanical construction, the fitting accuracy is higher than 94%. (3) At the same time, according to the analysis of the fitting results, the fitting effect of TBM mechanical thrust is the best among the four-parameter algorithm analysis fitting degrees, which is 99.3%. (4) The measured points are evenly distributed near the surrounding rock deformation surface calculated by the improved PSO-BP algorithm, and the fitting variance is 0.972. With the tunnel excavation, the settlement deformation of surrounding rock first increases rapidly, and then, the settlement deformation enters a slow growth stage and finally reaches a relatively stable state. (5) After the jam occurs, the tunnel is expanded from the telescopic shield or tail shield to the outer side of the shield to reduce the contact area between the shield and the surrounding rock to reduce the extrusion of the surrounding rock pressure on the shield.
