Behcet's disease (BD) is a rare multisystem chronic vasculitis of variable clinical presentation and unknown origin. Pulmonary involvement in BD is uncommon, with pleural effusion being an even rarer and difficult to diagnose manifestation. Herein, we report a challenging case of a young man who presented with recurrent pleural effusion and hepatic vein thrombosis and a recent history of papilledema with idiopathic intracranial hypertension. The patient was hospitalized for diagnostic and therapeutic thoracocentesis. Biochemistry and cytological analysis showed an exudative aspect of the collected pleural fluid with lymphocyte-dominated cytology. A multidisciplinary discussion was held, and thorough investigation was carried out to rule out malignant and infectious etiologies, among other differentials. During the second hospitalization, the patient complained of genital ulcers, which were verified to be recurrent along with oral ulcers. The diagnosis of BD was established based on the International Classification Criteria for BD, and the symptoms improved significantly upon using colchicine and immune-suppressive drugs.

Behçet’s disease adalah kelainan langka dan menyebabkan vaskulitis sistemik dengan manifestasi klinis yang melibatkan multiorgan. Kelainan ini umumnya ditemukan pada negara-negara di sepanjang jalur sutra terutama negara di Timur Tengah. Turki merupakan salah satu negara dengan prevalensi yang tinggi. Penyebab penyakit ini belum diketahui secara pasti, namun kombinasi faktor genetik berupa keterlibatan gen HLA-B51 dan faktor lingkungan berupa infeksi virus herpes simpleks atau bakteri spesies Streptococcus turut berperan dalam mencetus penyakit ini. Manifestasi klinis multiorgan yang paling sering ditunjukkan pada penyakit ini meliputi ulserasi oral, ulkus genital, uveitis, tromboflebitis superfisial dan lain sebagainya. Diagnosis kasus dilakukan berdasarkan kriteria klinis dengan instrumen yang digunakan saat ini merujuk pada International Criteria for Behcet’s Disease . Tatalaksana yang diberikan pada penyakit ini bergantung pada manifestasi klinis yang muncul dengan tujuan terapi untuk meningkatkan kualitas hidup pasien. Beberapa komplikasi dari penyakit ini seperti kebutaan akibat uveitis yang tidak segera mendapat pengobatan bahkan kematian akibat aneurisma arteri pulmonalis. Prognosis yang buruk ditemukan pada pasien dengan manifestasi klinis tertentu.

Background: Behcet’s disease (Silk Road disease), a rare immune-mediated multisystem inflammatory disorder described by intermittent oralaphthae and genital ulcer, backsliding uveitis, mucocutaneous, articular, gastrointestinal, neurological and vascular manifestations, with no cure.It is brought about by changes in the: a) arteries that flexibly blood to the body tissues b) veins that return the blood to the lungs, the rear of the eyes retina, brain, joints, skin and bowels. Case Presentation: A 55-year-old male patient was sensed with c/o joint agony in lower appendages, oral ulcer and scrotal ulcer. On physical assessment the patient was cognizant and oriented with B/L lower leg joint emanation. All lab examination including RA factor was within normal limits, with diminished Serum Vitamin D. HLA B51, ANA were checked and oral mucosal biopsy was done. The most punctual sign exhibited was oral disintegration, various shallow ulcer and scarcely any dissolved knobs in the scrotum. At that point the patient gave joint pain and numbness on right leg. On neurological assessment, a strange motor nerve conduction saw with right tibial neuropathy. At first, doubt with syphilis and tarsal tunnel disorder and following 7-8 days of affirmation, analyzed as Behcet's illness dependent on dermatological, rheumatologic and neurological signs. Treatment given was symptomatic and supportive with pain relievers, corticosteroid, antibiotics, IV fluids, PPI, vitamin supplement, laxative and local anaesthetic. Discussion:Without adequate data it's difficult to examine, in light of anomaly and standardized treatment are questionable at present. New information with respect to its immunopathogenesis, genetics will significantly help in the advancement of research center tests, diagnostic criteria and particularly in the decision of the best treatment
