Background: In medicine, radio-diagnosis and imaging is one of the vital tools used for the diagnostic purposes. In orthopaedic surgeries the use of intraoperative fluroscopy with C-arm is no doubt a beneficial technology but it comes with the occupational hazard of radiation exposure. This study therefore evaluates the awareness and practices of orthopaedic surgeons regarding occupational radiation exposure and its risks and the association of level of awareness and practices with various factors.Methods: A cross sectional study was conducted among orthopaedic surgeons using a pre-designed, pretested, semi-structured, self-administered questionnaire, disseminated online. Questions on awareness and practices of occupational radiation exposure and its risks were asked after obtaining consent. Data was analyzed using software statistical package for social sciences (SPSS) version 20 and a p value of <0.05 was considered to be statistically significant.Results: 150 completed forms showed that though majority (63%) of the study participants had adequate knowledge about occupational radiation exposure, the level of practices was only 12%. A statistically significant association (p<0.001) was found between years of experience and adequacy of knowledge and practices, regarding radiation exposure. Majority (91%) believed, adequate training and a health promotional approach could be beneficial.Conclusions: This study highlights the fact that occupational radiation exposure is an issue to be dealt with importance and thus the awareness and practices also needs to improve A health promotional approach with stringent rules needs to be taken to ensure adequate awareness and adequate practice among the orthopaedic surgeons.

Introdução: o uso da radiação ionizante na prática médica pode ocasionar eventos nocivos à saúde do indivíduo exposto. Diante disso, o emprego ético da radiação ionizante deve contemplar o que preconiza a legislação nacional e internacional, assim como os princípios de proteção radiológica e da bioética. O sofrimento moral relaciona-se à dimensão ética na prática da saúde, podendo afetar os profissionais das técnicas radiológicas. Objetivo: identificar as situações desencadeadoras de sofrimento moral nos profissionais das técnicas radiológicas em um serviço de radiologia convencional. Metodologia: trata-se de uma pesquisa qualitativa, descritiva e exploratória. Usou-se como técnicas de coleta de dados a observação participante, a entrevista semiestruturada e a validação consensual. Os dados foram analisados por meio da análise temática. Resultados e discussão: observou-se situações de sofrimento moral quando o profissional da técnica radiológica se deparava com pacientes em condições clínicas de se dirigir a unidade de diagnóstico e imagem, diante da não indicação clínica do exame radiológico, ante ao desrespeito dos princípios de proteção radiológica e perante a falta de autonomia para fazer cumprir os preceitos éticos do emprego da radiação ionizante. Conclusão: o desrespeito dos princípios legais, assim como dos princípios de proteção radiológica e bioéticos levam o profissional das técnicas radiológicas ao sofrimento moral.

Background In spite of frequent exposure to radiation, orthopaedic surgeons lack knowledge about radiation safety and do not comply with safety practices. We surveyed orthopaedic trainees and consultants in India to determine their knowledge and practices regarding radiation safety. Methods A questionnaire with 16 multiple choice questions was sent out using Google forms. We included practicing orthopaedic surgeons (consultants), fellows and trainees pursuing DNB, MS and D. Ortho courses across India. ResultsWe received 439/700 responses (62.7% response rate) from 233 (53.1%) consultants and 206 (46.9%) trainees. Only 71 (16.2%) were aware of the ALARA (As Low As Reasonably Achievable) principle. While lead aprons were always used by 379 (86.3%), thyroid shields were never used by 302 (68.8%) respondents. Knowledge about the ALARA principle was significantly associated with radiation safety practices. A significantly greater proportion of participants who were aware about the ALARA principle always used lead aprons (OR 1.15; 95% CI 1.0 to 1.2, p = 0.001) and thyroid shields (OR 2.00; 95% CI 1.0 to 3.7, p = 0.029) and had their dosimeters checked within the last 1 year (OR 1.69; 95% CI 1.0 to 2.8, p = 0.039) when compared to those who were not aware of the ALARA principle. Almost 99% respondents expressed interest in participating in a radiation safety training program. Conclusion A majority of the respondents were keen to obtain training in radiation safety. We believe that professional organizations and hospitals could initiate training programs for the orthopaedic community in India to improve their radiation safety knowledge and practices.
