Bedside Ocular Ultrasound for the Detection of Retinal Detachment in the Emergency Department

Abstract: Objectives: Acute retinal detachments (RD) can be difficult to diagnose and may require emergent intervention. This study was designed to assess the performance of emergency department ocular ultrasound (EOUS) for the diagnosis of RD.Methods: This was a prospective, observational study using a convenience sample of emergency department (ED) patients. Physicians performed EOUS for the diagnosis of RD prior to evaluation by an ophthalmologist. The criterion standard was the diagnosis of a RD by the ophthalmologi… Show more

“…Diagnosis of retinal detachments has been demonstrated using such a technique in the setting of one large ED with full specialty backup (12). A recent prospective, observational study demonstrated retinal detachment diagnosis with 100% sensitivity by emergency physicians (13). Although these studies were performed at single institutions on relatively small populations, the results suggest that ultrasonography can be used to diagnose retinal detachments with great utility.…”

“…Retinal detachments are funnel-shaped, thick projections, which may wave during ocular movement and attach at the optic disk (figure 12). 41 Vitreous detachments may appear like fine funnel shapes that wave freely and do not attach. The two may be indistinguishable; however, presence of echogenic material suggests injury and requires immediate further evaluation 40–44…”

“…For example, in vitreo-retinal surgery, preoperative planning with ultrasound can be used [5]. Recent studies have also shown the benefit of ocular ultrasound in emergency medicine, such as diagnosing retinal detachment [6,7]. There are some pathological characteristics that differentiate retinal detachment from other abnormalities such as vitreous hemorrhages [7].…”

There are some pathological characteristics that differentiate retinal detachment from other abnormalities such as vitreous hemorrhages [7]. Clinical manifestation of retina and vitreous detachments varies depending on where the adhesion is strongest [8].…”
