Summary. Objective: To determine the age-and sex-specific prevalence and determinants of retinal vein occlusions (RVOs) in a large population-based German cohort. Methods: The investigation included 15 010 participants (aged 35-74 years) from the Gutenberg Health Study. We determined the prevalence of RVO (central retinal vein occlusion [CRVO] and branch retinal vein occlusion [BRVO]) for the local population by assessing fundus photographs of 12 954 (86.3%; 49.8% women and 50.2% men) participants. Further, we analyzed the associations of RVO with cardiovascular, anthropometric, and ophthalmic parameters. Results: The weighted prevalences of RVO, CRVO, and BRVO were 0.40%, 0.08%, and 0.32%, respectively. Men were 1.7 times more frequently affected by RVO than were women. Prevalence of RVO was 0.2% in participants aged 35-44 and 45-54 years, respectively, 0.48% in those aged 55-64 years, and 0.92% in those aged 65-74 years. Of persons with RVO, 91.5% had one or more cardiovascular risk factor or disease vs. 75.9% of persons without RVO. BRVO was associated with arterial hypertension (odds ratio 2.69, 95% confidence interval 1.27-5.70) and atrial fibrillation (3.37, 1.24-9.12) and CRVO with higher age (7.02, 1.63-30.19) and a family history of stroke (4.64, 1.18-18.25). Median visual acuity (base 10 logarithm of minimum angle of resolution) was 0.2 in persons with RVO vs. 0.05 in those without. Conclusion: The prevalence of RVO in this German population was 0.4%, and men were 1.7 times more frequently affected than women. CRVO was associated with higher age and a family history of stroke, and BRVO was associated with arterial hypertension and atrial fibrillation.

Apolipoprotein E ( APOE) is a member of the apolipoprotein gene family. APOE is polymorphic with 3 main allelic types: ∊2, ∊3, and ∊4. Certain of these alleles have been associated with higher vascular risk. However, the association of APOE genotypes with retinal biomarkers and risk of retinal stroke is less clear. This study evaluated the role of APOE polymorphisms in retinal vein occlusion (RVO). In the present study, 2-point mutations coding amino acid residues 112 and 158 were amplified using the polymerase chain reaction (PCR) from DNA extracted from Tunisian participants. APOE genotypes were determined by multiplex PCR followed by molecular hybridization. Eighty-eight patients (26 women and 62 men) and 100 age- and gender-matched healthy participants were enrolled. The statistical study revealed a higher frequency of the ∊4 allele in patients as compared to controls (27.3% vs 9%) with a significant association of the ∊4 allele with the disease ( P < 10, P < 10, odds ratio [OR] = 3.8, 95% confidence interval [CI] = 2.1-6.8). The frequency of the ∊3 allele was significantly lower in the patients with RVO compared to the controls (60.2% vs 82.5%, respectively; P < 10, P < 10, OR = 0.32, 95% CI = 0.19-0.53). The ∊3 allele seems to be protective against the disease. There was no association between the APO ∊2 allele and RVO. The association of APOE allele and genotype with RVO requires further investigation in different populations.

Purpose: To identify ocular comorbidity in systemic hypertension. Methods:Research findings include data of 566 patients with RVO ( retinal vein occlusion) 408 and 158 patients with the central retinal vein occlusion (CRVO) and branch retinal vein occlusion (BRVO) respectively, representing a cases and and 566 controls, all aged 31 years and older. Excluded from case and control group were persons with degenerative changes of retina and ocular inflammation. At the baseline examination blood pressures was measured and were tested fibrinogen, recalcification time, plasma tolerance to heparin, prothrombin ratio for evaluation of blood coagulability. A statistical analysis was conducted by a commercially available statistical software package.Results: Among our cases hypertension was more prevalent with an elevated systolic and diastolic blood pressures comparing to controls.Hypercoagulability represented by elevated prothrombin ratio has shown a close association with age and RVO, evidencing higher risk of vasoocclusion in older persons with hypercoagulability. It was found that higher blood pressure: systolic blood pressure (OR, 8.49; 95% CI, 4.81 to 15.13) and diastolic blood pressure (OR, 9.37; 95% CI, 6.34 to 13.89); prothrombin ratio (OR, 4.0; 95% CI, 2.17 to 7.7) after adjusting for age and sex, plays a significant role in the development of RVO. Assessing an impact of systemic hypertension duration on RVO frequency it was evidenced direct relationship indicating increased cases of RVO in patients suffering from systemic hypertension 5-10 years and more obvious impact in case of longer duration-more than 10 years. Conclusions:The study emphasized the need for enhanced collaboration between specialties to ensure appropriate management of patients with systemic hypertension and ocular comorbidity in order to prevent occurrence of retinal vein occlusion.
