BackgroundCold spells and heatwaves increase mortality. However little is known about the effect of heatwaves or cold spells on cardiovascular morbidity. This study aims to assess the effect of cold spells and heatwaves on cardiovascular diseases in a Mediterranean region (Catalonia, Southern Europe).MethodsWe conducted a population-based retrospective study. Data were obtained from the System for the Development of Research in Primary Care and from the Catalan Meteorological Service. The outcome was first emergency hospitalizations due to coronary heart disease, stroke, or heart failure. Exposures were: cold spells; cold spells and 3 or 7 subsequent days; and heatwaves. Incidence rate ratios (IRR) and 95% confidence intervals were calculated using the self-controlled case series method. We accounted for age, time trends, and air pollutants; results were shown by age groups, gender or cardiovascular event type.ResultsThere were 22,611 cardiovascular hospitalizations in winter and 17,017 in summer between 2006 and 2013. The overall incidence of cardiovascular hospitalizations significantly increased during cold spells (IRR = 1.120; CI 95%: 1.10–1.30) and the effect was even stronger in the 7 days subsequent to the cold spell (IRR = 1.29; CI 95%: 1.22–1.36). Conversely, cardiovascular hospitalizations did not increase during heatwaves, neither in the overall nor in the stratified analysis.ConclusionsCold spells but not heatwaves, increased the incidence of emergency cardiovascular hospitalizations in Catalonia. The effect of cold spells was greater when including the 7 subsequent days. SummaryBackgroundOlder subjects with type 2 diabetes mellitus (T2DM) have differential characteristics compared with middle‐aged or younger populations, and require tailored management of the disease.AimsTo evaluate how clinical characteristics, degree of control of glycaemia and cardiovascular risk factors, presence of chronic complications and treatments differ between older T2DM patients and younger adults.MethodsCross‐sectional study using data from a population‐based electronic database. We retrieved data from 318,020 patients ≥ 30 years diagnosed with T2DM, attended during 2011 in primary care centres in Catalonia, Spain. We performed descriptive and comparative analyses stratified by gender and age subgroups: ≤ 65, 66–75, 76–85 and >85 years.ResultsBoth men and women across older age subgroups (> 65 years) had longer diabetes duration than younger adults (8.0 vs. 5.6 in men and 8.4 vs. 6.9 years in women; p < 0.001), but better glycaemic control (mean glycated haemoglobin 7.1 vs. 7.7 in men and 7.1 vs. 7.4 in women; p < 0.001), and better combined control of different cardiovascular risk factors (p < 0.001). Moreover, older patients were more likely to achieve glycaemic targets irrespective of having cardiovascular disease. The use of oral antidiabetics decreased with increasing age, and insulin in monotherapy was more frequently prescribed among patients in the older age subgroups. Diabetes‐related complications were more frequent in men of all group ages. In the older age subgroups, patients of both sexes had a longer duration of T2DM but better glycaemic control. In this context, the prevalence of diabetic retinopathy decreased unexpectedly with increasing age.ConclusionControl of glycaemia and cardiovascular risk factors was better among older T2DM patients. There is a need for prospective studies to quantify the weight of risk factors in each complication to adapt the therapeutic and care approaches in elderly people.

Objective: To analyse the association between MEDEA (Mortality in small areas of Spain and socioeconomic and environmental inequalities) index and mortality in urban and rural areas of Catalonia. Methods: An ecological study based on the analysis of census section. The data source used for census section and variables to calculate the MEDEA index was the census (2001). Mortality data were obtained from System for the Development of Research in Primary Care (SIDIAP). The census sections were classified as rural or urban. The association between mortality and the socioeconomic index was analysed as categorical variable (quintiles). Poisson models were fitted to study the association between MEDEA index and mortality. Analysis was done with the STATA software, version 12. Results: In January 2009, a total of 4,526,071 adults (> 14 years old) were assigned to ICS (Institut Català de la Salut ) primary healthcare centres. The identified population lived in 5,214 census sections out of a total of 5,222 existing areas, from which 4096 (78.5%) were urban. The association between MEDEA quintiles and mortality was significant for urban areas excluding Barcelona: excess mortality was 5% (IRR = 1.05 IC 95% 1.01-1.10), being higher in urban areas (IRR = 1.11 95% CI 1.08 to 1.15) and even higher in Barcelona (IRR = 1.24 95% CI 1.18 to 1.31). This association was not significant for rural areas (IRR = 0.95 CI 95% 0.88-1.02). Conclusions: Socioeconomic deprivation, measured with the MEDEA index, was related with an increase in total mortality in urban areas of Catalonia. In rural areas there was no association with mortality.
