Barriers to the implementation of isoniazid preventive therapy for tuberculosis in children in endemic settings: A review

Abstract: Isoniazid preventive therapy is one of the key interventions in reducing the risk of active disease among children exposed to tuberculosis. However, initiation and maintenance of this treatment is poor in many areas. This review summarises the existing literature on barriers to implementation of isoniazid preventive therapy for tuberculosis in children in endemic settings. MEDLINE, EMBASE and CINAHL databases were used to search for primary research studies published between 1998 and 2018, specifically mention… Show more

“…In this study, there was a trend of virally non‐suppressed CLHIV being less likely to initiate IPT suggesting that they could have been sicker at enrolment, thus their providers may not have felt confident in ruling out active TB. The impact of caregiver barriers, such as the cost of travel for IPT services and the perception of minimal risk in asymptomatic children [ 28 ], may have also contributed.…”

“…Attending high-volume HIV clinics (aRR = 2.82; 95% CI 1.20-6.62) was independently associated with IPT non-initiation. IPT non-initiation had a trend of being higher among those enrolled in the period -2019 versus 2015 0.98-3.73) and those who were HIV virally non-suppressed (aRR = 1.90; 95% CI 0.98-3.71). Being enrolled in -2019 versus 2015 1.01-1.96) was independently associated with IPT non-completion.…”

“…Of several factors that contribute to the low uptake of TPT in the household contact groups, cost of travel to health facilities; perceived lack of need for TPT in children among parents; low level of knowledge among healthcare workers; stigma; other competing priorities of the family; and medication related challenges such as pill burden stand out as common barriers. 11 12 Other factors include fear of side effects, 7 lack of supplies and poor recording and reporting practices. 13 Common themes with the barriers include access and information, which may be better addressed with community-based interventions.…”

“…Although the proper implementation of international standards for TB prevention could prevent the deaths of thousands of children and adolescents around the globe [7], initiation and maintenance of preventive treatment is poor [8]. Various countries have reported non-compliance with policies aimed at medical management of TB contacts [9].…”
