Barriers and facilitators of implementing proactive deprescribing within primary care: a systematic review

Abstract: Objective Proactive deprescribing – identifying and discontinuing medicines where harms outweigh benefits – can minimise problematic polypharmacy, but has yet to be implemented into routine practice.
