Barium Enema Revisited in the Workup for the Diagnosis of Hirschsprung's Disease

Abstract: Objective: To analyse the diagnostic capacity of barium enema (BE) in the diagnostic investigation for Hirschsprung's disease (HD) was analyzed for transition zone (TZ) identification and rectosigmoid index (RSI) ≤1.0 determination. Patients and Methods: BE images were analyzed retrospectively by 2 examiners and the results were compared with the histopathology of rectal biopsies. Results: TZ identification … Show more

“…When the whole colon and rectum are aganglionic and spasmodic, rectal ampulla may be di cult to identfy in barium enema, even for experienced radiologists. Transition zone is regarded as an effective radiographic sign to identify the extent of aganglionic bowel in "segmental" HD and could therefore facilitate surgical planning [21]. In our study, the two radiologists noted these occurrences in 12 (12/44, 27%) and 26 (26/44, 59%) patients, and both of these values were higher than that reported in a previous study of TCA (2/17, 12%) [7].…”

“…However, there are still some problems associated with water-soluble contrast; these include the risk of dehydration and a lack of research [20]. Barium enema is still the most widely used method for diagnosing TCA in most centers [19,21].…”

“…9 The aganglionic segment is due to failure of migration of neural crest cells (precursors of enteric ganglion cells) during organogenesia. 15 The diagnosis of HD is usually based on a combination of the presenting symptoms, radiological studies, rectal manometry and histological features of rectal biopsy. 16…”

“…Different radiologists may choose different radiographs to record rectosigmoid index ≤ 1 in a patient, which would influence judgment.
