Balloon-assisted coil embolization (BACE) of a wide-necked aneurysm of the inferior pancreaticoduodenal artery

Abstract: Background: Aneurysms of the pancreaticoduodenal arcades are an uncommon pathology, with a prevalence of 2%, and could be congenital or acquired. Treatment of visceral aneurysms is therefore generally recommended when the aneurysmal sac equals or exceeds 2 cm. Wide-necked (> 4 mm) and main artery branch aneurysms represent a challenge for conventional endovascular coil embolization due to the risk of coil migration. Main body: This case describes the technical feasibility of balloon-assisted coil embolization … Show more

“…PAs with a neck width surpassing 4 mm present considerable challenges for conventional coil embolization, including risks of coil displacement and partial occlusion. These complications could result in persistent blood flow into the aneurysm and a heightened risk of rupture [21,22]. Our patient's PA had a neck sufficiently wide to pose these risks, influencing our decision in favor of stent-graft placement.…”

“…Balloon-assisted coil embolization strategy is also employed in case of wide neck saccular aneurysms [ 55 ]. In this situation, a microcatheter is placed within the aneurysm sac while a parallel balloon catheter is inflated along the parent vessel across the aneurysm neck to avoid coils prolapsing into the target artery lumen [ 56 ].…”

“…Early bleeding is often caused by the technical failure of appropriate hemostasis during the operation. In contrast, late bleeding is mainly caused by POPF, intra-abdominal abscess, and bile leak [ 6 , 7 ].…”
