Bali Belly:Salmonella senftenbergFound in an Infected Ovarian Endometrioma

Abstract: Salmonella is an extremely rare cause of an infected endometrioma. We present a case of a 30-year-old immunocompetent woman presenting with fevers and abdominal pain, on a background of prior endometriosis. Initial antibiotic treatment for pelvic inflammatory disease failed, and the patient progressed to septic shock requiring surgical evacuation of an infected ovarian endometrioma. Microbiological samples from stool, ovary, and peritoneal fluid revealed infection with Salmonella senftenberg. The likely diagno… Show more

“…6 The latter is possible via direct bacteremia or by the bacteria breaching the intestinal cell wall and entering macrophages which can lead to the involvement of secondary locations of Salmonella. 7 In our case, the Salmonella bacteria could be cultured in blood cultures, a cervical swab, and a stool culture. Our primary hypothesis is contamination during surgery with subsequent triggering of an invasive infection.…”

“…Organisms can reach the organs of the female genital tract through three pathways: ascending vaginally, via direct contact with an inflamed bowel or through hematogenous dissemination 6 . The latter is possible via direct bacteremia or by the bacteria breaching the intestinal cell wall and entering macrophages which can lead to the involvement of secondary locations of Salmonella 7 . In our case, the Salmonella bacteria could be cultured in blood cultures, a cervical swab, and a stool culture.…”

“…Infected endometrioma is a rare entity, and current literature is limited to a small number of case reports [ 1 , 6 , 8 , [9] , [10] , [11] , [12] , [13] ]. Lower abdominal pain and fever are generally expected clinical findings.…”

“…However, in some cases, the insidious onset of symptoms may cause patients to be admitted to the hospital late with signs of peritonitis in a preseptic state. Early detection of infection is vital, as it may be complicated by rupture and can be fatal [ 1 , 8 ].…”

“…As a result, vaginal flora, including anaerobic organisms, Gardnerella vaginalis and Mycoplasma hominis and genitalium can cause various gynecological lesions such as endometritis, salpingitis oophoritis, or abscesses [ 14 ]. In the international literature, there have been reported some clinical cases with infected endometriotic cysts that had a medical history of previous infections, such as gastrointestinal infection due to Helicobacter cinaedi or Salmonella [ 15 , 16 ] and genital tract infections [ 17 ]. These examples support a theory that ovarian abscesses in endometriomas can be caused by the hematogenous or lymphatic spread of bacteria [ 18 ].…”
