ObjectivesThe purpose of this study was systematically and quantitatively to assess the value of the neutrophil-to-lymphocyte ratio (NLR) for the diagnosis of neonatal sepsis by systematic review and meta-analysis.DesignSystematic review and meta-analysis.MethodsEight major databases, including The Cochrane, PubMed, Embase, Web of Science, CNKI, Wanfang, China Biomedical Literature Database and VIP Database, were systematically searched for NLR diagnoses of neonatal sepsis from inception to June 2022. Two investigators independently conducted the literature search, screening, data extraction and quality evaluation with the Quality Assessment of Diagnostic Accuracy Studies-2 checklist. Statistical analysis was performed using Review Manager V.5.3, Stata V.16.0, R (V.3.6.0) and Meta-DISC V.1.4.ResultsA total of 14 studies comprising 1499 newborns were included in this meta-analysis. With a cut-off value ranging from 0.1 to 9.4, the pooled sensitivity of the NLR in the diagnosis of neonatal sepsis was 0.74 (95% CI: 0.61 to 0.83), the pooled specificity was 0.88 (95% CI: 0.73 to 0.95), the positive likelihood ratio (LR+) was 6.35 (95% CI: 2.6 to 15.47), the negative likelihood ratio (LR−) was 0.30 (95% CI: 0.19 to 0.46), the diagnostic OR (DOR) was 12.88 (95% CI: 4.47 to 37.08), area under the curve (AUC) was 0.87 (95% CI: 0.84 to 0.89). In the subgroup analysis of early-onset neonatal sepsis, the pooled sensitivity was 0.75 (95% CI: 0.47 to 0.91), the pooled specificity was 0.99 (95% CI: 0.88 to 1.00), the LR+was 63.3 (95% CI: 5.7 to 696.8), the LR−was 0.26 (95% CI: 0.10 to 0.63), the DOR was 247 (95% CI: 16 to 3785) and the AUC was 0.97 (95% CI: 0.95 to 0.98).ConclusionsOur findings suggest that the NLR is a helpful indicator for the diagnosis of early neonatal sepsis, but it still needs to be combined with other laboratory tests and specific clinical manifestations.

Background: Neonatal period forms basic foundation which influences the future health of a child. It is often complicated by neonatal sepsis. In resource limited settings especially in rural and tribal areas with limited laboratory services it will be not possible to have all investigations. Study aims to determine utility of simple neutrophil to lymphocyte ratio as a marker of sepsis in neonates within the first week of life.Methods: Retrospective descriptive study in a tertiary care hospital in Kochi. Objective of this study was to analyse neutrophil to lymphocyte ratio in babies with sepsis to that of babies without sepsis. Data of 300 babies were collected which included 150 babies with sepsis and 150 babies with no sepsis. Records showing complete history, clinical diagnosis, appropriate investigation and management details of term babies within 7 days of life were included. Preterm babies, Babies with congenital malformations, outborn babies, babies with incomplete records were excluded.Results: In this study, mean value of neutrophil to lymphocyte ratio in babies with sepsis were 3.73 compared to 1.22 in babies with no sepsis. For babies with sepsis±1SD values were 5.21 and 2.25 respectively and for babies with no sepsis were 1.74 and 0.70. For babies with sepsis±2 SD values 0.77 and 6.69 respectively and 0.18 and 2.26 for babies with no sepsis.Conclusions: Mean values of NLR values were significantly higher in those with sepsis than in those without sepsis NLR ratio is higher in cases with sepsis (3.728±1.483) compared to the cases with no sepsis (1.217±0.521). A value more than 2.25 can be indicator of sepsis.

Among all neonatal diseases, neonatal pneumonia is one of the common diseases. Multi-slice spiral CT and chest X-ray imaging images are used to diagnose and study neonatal pneumonia in hospitals and treat neonates in China. Pneumonia is of great significance. This article retrospectively analyzes the CT and chest X-ray images of neonatal pneumonia, and divides the cases into two groups according to their invasiveness: pre-invasive and micro-invasive group, invasive group, and the size of the air-containing cavity Analyze the multiple occurrence and distribution position; observe the air-containing cavity structure morphology under the microscope. All patients were scanned by the chest multi-slice spiral CT scanner, and the 1m or 0.625m thin-slice images were sent to the post-processing workstation for statistics and analysis of relevant data. This article discusses the entire calculation process from projection data preprocessing to spiral interpolation to image reconstruction of neonatal pneumonia. The effects of the two interpolation modes on the reconstruction results are compared. A practical multi-layer spiral cone beam algorithm is given. An important aspect of the performance of neonatal pneumonia image performance and its generation mechanism are analyzed. As an equipment manufacturer, it provides a basis for the improvement and correction of machine performance. This article analyzes the real-time fluorescence quantitative PCR value of neonatal Mycoplasma pneumoniae and its gender, Relevance of age, heat history, heat type, shortness of breath, wheezing, atelectasis, pleural effusion and ventilator ventilation treatment, and the study subjects were divided into two groups according to whether they were treated with macrolide antibiotics. After heterogeneity, the relationship between the severity of the disease and the pathogen load and inflammation in the two groups was studied.
