Bacterial decolonization to prevent acute radiation dermatitis: A randomized controlled trial.

Abstract: LBA12003 Background: Radiation dermatitis (RD) secondary to radiation therapy (RT) to treat cancer reduces quality of life (QoL) and can lead to treatment interruption. The exact etiology of RD is unknown, and bacteria play a role in other inflammatory dermatoses. As our group recently showed nasal colonization with Staphylococcus aureus (SA) prior to RT was an independent predictor of grade ≥2 RD, we conducted a randomized controlled trial evaluating efficacy of bacterial decolonization (BD) to prevent RD an… Show more

“…39 The low opinion on topical antibiotics is somewhat surprising, as many international guidelines recommend antibiotics either prophylactically or therapeutically since there is evidence for positive effects of bacterial decolonization on RD. 40,41 Aloe vera with its anti-inflammatory properties showed some protective effect in a RCT versus mild soap, 42 but has not proved to reduce severe RD, 43 and was reported inferior to other controversial agents. 44 Given the lack of evidence, the benefits of aloe vera appear to be slightly overestimated among respondents.…”

“…Consequently, these effects may lead to severe radiodermatitis. Kost et al 6 showed that decolonization of the skin before and during radiation treatment prevents severe radiodermatitis. On the basis of our results, a possible explanation for the positive effect of chlorhexidine body cleanser could be the prevention of bacterial overgrowth in these patients, who due to a low baseline level of commensals are at higher risk for severe radiodermatitis.…”

“…RCTs were recently published on interventions already identified through the present review, such as Mepitel film, 258 topical corticosteroids, 259 , 260 heparinoid, 261 photobiomodulation, 257 , 262 , 263 aloe vera gel, 264 calendula, 256 and others. Additional RCTs were identified on new interventions not previously explored before September 2020, such as the use of compound Kushen injection, 265 epigallocatechin-3-gallate, 201 , 266 bacterial decolonization, 267 and others. For example, in a population of 81 breast cancer patients, Siddiquee et al.…”
