Intracardiac QRS Electrogram Width—An Arrhythmia Detection Feature for Implantable Cardioverter Defibrillators: Exercise Induced Variation as a Base for Device Programming

Abstract: Delivery of inappropriate therapy of implantable cardioverter defibrillators (ICD) due to inaccurate arrhythmia detection represents a major clinical problem. Different arrhythmia detection criteria such as the "stability" of the cycle length or the suddenness of "onset" of tachycardia have been implemented in ICD software to prevent inappropriate therapy. The new Medtronic model 7223Cx ICD offers an additional detection parameter (QRS width), which reflects changes in the duration of ventricular depolarizatio… Show more

“…EGM width had some limitations, as for example discrimination of SVT from VT in patients with bundle branch block. Furthermore, this early algorithm needed careful and experienced monitoring and device programming to provide a sufficient performance 8,10,19,20 . During spontaneous episodes, the Wavelet algorithm has been shown to be superior to EGM width for tachyarrhythmia discrimination 21 .…”

“…Optimal clinical performance of these algorithms is facilitated by patient tailored programming. [16][17][18][19][20] Ventricular EGM morphology algorithms for SVT discrimination were added [21][22][23][24] and operate by monitoring EGM morphology and detecting whether a tachycardia has changed morphology in comparison with usual rhythm.…”

“…Later devices also examined abruptness of tachycardia onset (“onset”) and beat‐to‐beat interval variability (“stability”) to avoid inappropriate shocks for sinus tachycardia or atrial fibrillation 4–6 . A simple morphology algorithm based on electrogram (EGM) width has been used in ICDs to reduce inappropriate detection 7 . Recently, more complex EGM template‐matching algorithms such as “Wavelet,” 8 “Morphology Discrimination,” 9 and “Rhythm ID” 10,11 have been applied to further improve discrimination of VT from SVT.…”
