Coronavirus Disease-19 (COVID-19) is caused by the Severe Acute Respiratory Syndrome Coronavirus (SARS-CoV-2) as part of the novel coronavirus. Clinical monitoring and appropriate treatment strategies are essential to reduce case fatality. Various examination parameters to support diagnosis such as ESR and CRP in assessing the response of a systemic inflammatory process. C-reactive protein (CRP) levels can be used in the initial diagnosis of pneumonia and patients with severe pneumonia have high CRP levels. Erythrocyte Sedimentation Rate (ESR) as a non-specific inflammatory response examination which has increased in various physiological disorders such as infection and inflammation. The purpose of this study was to determine the levels of ESR and CRP as biomarkers of inflammation in patients with COVID-19 at RSPTN UNHAS, Makassar. This study involved 32 subjects who met the inclusion criteria for positive RT-PCR COVID-19, age 21-70 years and signed of informed concent. Erythrocyte Sedimentation Rate Check by Auto ESR Analyzer using the Westergreen method and C-reactive protein Check by qualitative test. The result of this research that the average age of the subjects was 51 years consisting of 14 (56.2%) female subjects and 18 (43.8%) male subjects. The median value of CRP examination is 48.00 mg/dL and the median value of ESR is 63.00 mm/hour. 2. There is a significant difference between the CRP values in male and female subjects. Introduction: The current incidence of the novel coronavirus disease has shown only small reductions of cases and has become a major public health challenge. Development of effective vaccines against the virus is still being encouraged such as multi-epitope vaccines designed from the components of SARS-CoV-2 including its spike, nucleocapsid and ORF1a proteins. Since the addition of adjuvants including HABA protein and L7/L12 ribosomal are considered helpful to increase the effectiveness of the designed vaccine, we proposed to design multiepitope vaccines by two different adjuvants. Methods: We used the IEDB server to predict BCL and TCL epitopes that were characterized using online tools including VaxiJen, AllPred and IL-10 Prediction. The selected epitopes were further constructed into multiepitope vaccines. We also added two different adjuvants to the vaccine components in order to increase the effectiveness of the vaccines. The 3D-structured vaccines were built using trRosetta. They were further docked with different Toll-like-receptors (TLR 3, 4 and 8) and the entry receptor of SARS-CoV-2, ACE2 using ClusPro, PatchDock and refined by FireDock. All structures were visualized by USCF Chimera and PyMOL. Results: In this study, we succeeded in designing two different candidate vaccines by the addition of HABA protein and L7/L12 ribosomal as adjuvants. The two vaccines were almost equally good in terms of their physicochemical properties and characteristics. Likewise, their strong interactions with TLR3 4, 8 and ACE2 show the lowest energy level of both was estimated at more than -1,000. Interactions of vaccines with ACE2 and TLRs are essential for activation of immune responses and production of antibodies. Conclusion: The two designed and constructed multiepitope vaccine have good characteristics and may have the potential to activate humoral and cellular immune responses against SARS-CoV-2. Further research is worth considering to confirm the findings of this study.

Lessons and New Perspectives: Is Convalescent Plasma Therapy Effective on Covid-19 Patients?

Objective: Recently, convalescent plasma (CP) therapy has shown promising evidence in the treatment of several serious contagious diseases, including SARS-CoV, Influenza and Ebola. We conducted a systematic review to extract data about using CP treatment for COVID-19 patients and it’s effectively. Methods: The retrieval of studies was conducted according to Cochrane Collaboration and from electronic databases including PubMed, Medline, and others (medRxiv and BioRxiv). Searching of the available evidence concerning CP treatment of COVID-19 patients was conducted in journal articles published between December 2019 and October 2020. The articles were further screened based on inclusion and exclusion criteria to identify the high-quality studies for analysis. Results: A total of 18 CP studies were included in this review. We found variance regarding the effectiveness of CP in the reduction of mortality rate, length of stay, and increased discharging rate. Several findings show CP therapy is effective in increasing viral negativity, neutralizing antibodies to recipients, does not cause harmful adverse reactions and in some cases can improve clinical symptoms. This therapy is presently considered effective for generating good clinical outcomes when given early in the course of the disease. Conclusion: The effectiveness of CP in terms of mortality, length of stay, and increased discharging patients is still debatable. However, CP therapy is effective in increasing the negativity of SARS-CoV-2 test, neutralizing antibody titer and is safe so it can be considered for COVID-19 patients. CP should not be given in the initial disease course but is recommended for the early disease course.
