Background: Among the non-communicable diseases prevalent among old age home inmates, hypertension is one of the diseases which if not diagnosed and treated adequately, could lead to high mortality and morbidity due to limited resources. The objective of this study is to find out the prevalence of hypertension among the old age home inmates and also its association with psychosocial morbidity among them. Methods: This community based cross sectional descriptive study was done among 250 inmates of selected old age homes in Kancheepuram district. A total of 8 old age homes were randomly chosen and universal sampling was done in each old age home to obtain the required sample size. Structured questionnaire was used to collect data and psychosocial morbidity among the inmates was assessed by Depression, Anxiety and Stress scale (DASS 21). Data analysis was done by SPSS software version 21. Results: The prevalence of hypertension and prehypertension was found to be 72.8% and 10% respectively and the prevalence of psychosocial morbidity was found to be high among the study participants (depression– 68.4, anxiety–46.8, stress– 50.4). Statistically significant association was found between uncontrolled hypertension and psychosocial factors (anxiety and stress). About 27% of the hypertensive was unaware of free medication offered for hypertension at Government health centres. Conclusions: This study highlights the need for setting up of specialized geriatric clinics at Government Health Centres to provide speedy medical services and supply of medication for the elderly. Health education has to be given to both the inmates and care takers about chronic diseases like hypertension.

Objective: Despite its sensitivity to light, atenolol has been manufactured as a film-coated tablet with normal blister packaging by several pharmaceutical companies in Bangladesh. The aim of this study was to investigate the extent of light-induced potency degradation of a randomly selected film coated brand of atenolol.Methods: Atenolol tablets were exposed to different lighting conditions such as normal room light, direct sunlight and different incandescent lights (25W and 40W). Subsequently, UV spectroscopy technique was employed to determine the relative reduction of light absorbance compared to their respective controls. Thereafter, photolytic degradation was calculated by means of the potency reduction of tablets.Results: In all lighting conditions, atenolol tablets underwent exposure dependent gradual decrease in potency. Except for normal room light condition, a significant decrease in potency was found even after 4 to 6 h of exposure to all lighting conditions. After 6 h, potency reduction was found at 40-47%, 26-38% and 34-36% in the samples exposed to direct sunlight, 25 W bulb, and 40 W bulb respectively. Although the shelf life of the film coated tables was 2 y, surprisingly, statistically significant reduction in potency was observed within only 30 d in room light condition.Conclusion: In order to protect from light, blister packaging is not sufficient for film coated atenolol tablets. Photo-stability of all brands of atenolol must be ensured either by protective packaging materials or by optimizing the formulations.

Evaluating the Cost-Effectiveness of Different Groups of Hypertension Therapy: A Pharmacoeconomic Study

Objective: The study objective is to evaluate among the different groups of antihypertensive for their cost-effectiveness and to provide information about the effective management of hypertensives both clinically and economically. Methods: This study involves 81 patients aged 18–80 years on antihypertensive treatment who were enrolled from December to May with daily dose calculation. It is performed to analyze cost-effectiveness in the management of clinical condition. Data collection form to be entered with age, sex, food habits, occupational status, clinical data, and drugs was used for treatment. Incremental cost-effectiveness ratio is calculated based on the incremental cost for “per mmHg” reduction and cost for “per patient” reaching target blood pressure (BP). Result: Angiotensin receptor blockers (ARBs) show a significant average reduction of systolic BP (SBP) and beta-blockers (BBs) show an average reduction of diastolic BP (DBP). Angiotensin-converting enzyme inhibitors (ACEIs) are cost effective in SBP and calcium channel blockers are cost effective in maintaining DBP. Conclusion: ARBs, BBs, and ACEIs show the effective management of hypertensives both clinically and economically.
