Behçet’s disease (BD) is a chronic systemic inflammatory vasculitis of unknown cause that is recognized worldwide. Cardiovascular system involvement is rare and the most common manifestation of endocardiac involvement in BD is aortic regurgitation (AR). The ultrasonic manifestations of BD- related AR such as echo-free spaces and vegetation-like lesions, can be misdiagnosed with infective endocarditis and bacterial vegetations [1]. This article presents a rare case of avulsion of aortic valve (AV) commissure associated with BD, who experienced hypotension and shock supported with extracorporeal membrane oxygenation.
