Avulsion Fracture of the Calcaneal Tuberosity: Case Report and Literature Review

Abstract: Avulsion fractures of the calcaneal tuberosity are predominantly seen in patients with poor bone quality, the commonly used lag screw fixation might not be strong enough even with bony fragments of sufficient size. We present a case of a closed displaced avulsion fracture of the calcaneal tuberosity due to blunt trauma to the calf in a 74-year-old female. Open reduction and internal fixation with two 3.5-mm cannulated cortical screws with washers was performed, and anatomic reduction was achieved. Without furt… Show more

“…The variations in injury mechanisms between the two groups could impact the occurrence of TFCC injuries [28]. Additionally, avulsion fractures in areas like calcaneal tuberosity exhibit peak incidence in elderly patients and are associated with decreased BMD [41,42]. Similarly, in osteoporotic patients, when TFCC injury occurs predominantly on the volar aspect of the forearm during a fall with an outstretched arm, where torsional forces are applied, it is possible that the compromised bone quality may lead to the occurrence of fragile fractures rather than ligament injuries.…”

“…Lag screw fixation is a most common surgical procedure [ 13 , 14 ]. However, this procedure is difficult to fix bone fragments rigidly enough to resist the tensile force of the gastrocnemius–soleus complex when bone fragments are small and osteoporotic [ 1 , 7 , 8 ]. Squires et al.…”

“…Fracture of the calcaneus is a common fracture usually caused by falling from a height, landing on the heel, and vertical force to the heel. As a result, excessive swelling of the heel, the shallowing of the posterior sulcus of the ankle, and the compression as well as pain of the heel occur [14,15]. The patients with fracture of the calcaneus with the calcaneal articular surface unaffected and inconspicuous displacement can be performed with nonoperative external fixation [16].…”
