As the total volume of mammograms in Dubai is increasing consistently, it is crucial to focus on the process of dose optimization by determining dose reference levels for such sensitive radiographic examinations as mammography. This work aimed to determine local diagnostic reference levels (DRLs) for mammography procedures in Dubai at different ranges of breast thickness. A total of 2599 anonymized mammograms were randomly retrieved from a central dose survey database. Mammographic cases for screening women aged from 40 to 69 years were included, while cases of breast implants and breast thickness outside the range of 20–100 mm were excluded. Mean, median, and 75 percentiles were obtained for the mean glandular dose (MGD) distribution of each mammography projection for all compressed breast thickness (CBT) ranges. The local DRLs for mammography in Dubai were found to be between 0.80 mGy and 0.82 mGy for the craniocaudal (CC) projection and between 0.89 mGy and 0.971.8 mGy for the mediolateral oblique (MLO) projection. Local DRLs were proposed according to different breast thicknesses, starting from 20 to 100 mm. All groups of CBT showed a slight difference in MGD values, with higher values in MLO views rather than CC views. The local DRLs in this study were lower than some other Middle Eastern countries and lower than the standard reference levels reported by the International Atomic Energy Agency (IAEA) at 3 mGy/view.

The purpose of this study was to investigate whether the lesion risk score provided by an AI system is influenced by the selection of exposure parameters. A breast phantom which contains a lesion, was imaged with digital mammography with different imaging conditions. The tube voltage, the dose level and the anode-filter combination were varied based on an exposure obtained with automatic exposure control. The organ dose for each image was extracted from the DICOM header. The images were analyzed with an AI system, which provided a lesion risk score (suspicion for malignancy) for each exposure condition. Correlations between the lesion risk score and the exposure conditions were investigated. The results of the study showed that the organ dose had a strong impact on the lesion risk score. Reducing the organ dose to a low level resulted in that the AI system no longer detected the lesion. Images of suboptimal quality may result in inaccurate AI system performance. In our preliminary analysis, the breast phantom and the lesion were proven to be realistic enough for being analyzed by the AI system.

In radiography, much valuable associated data (metadata) is generated during image acquisition. The current setup of picture archive and communication systems (PACS) can make extraction of this metadata difficult, especially as it is typically stored with the image. The aim of this work is to examine the current challenges in extracting image metadata and to discuss the potential benefits of using this rich information. This work focuses on breast screening, though the conclusions are applicable to other modalities. The data stored in PACS contain information, currently underutilised, and is of great benefit for auditing and improving imaging and radiographic practice. From the literature, we present examples of the potential clinical benefit such as audits of dose, and radiographic practice, as well as more advanced research highlighting the effects of radiographic practice, e.g. cancer detection rates affected by imaging technology. This review considers the challenges in extracting data, namely, • The search tools for data on most PACS are inadequate being both time-consuming and limited in elements that can be searched. • Security and information governance considerations • Anonymisation of data if required • Data curation The review describes some solutions that have been successfully implemented. • Retrospective extraction: direct query on PACS • Extracting data prospectively • Use of structured reports • Use of trusted research environments Ultimately, the data access process will be made easier by inclusion during PACS procurement. Auditing data from PACS can be used to improve quality of imaging and workflow, all of which will be a clinical benefit to patients.
