Avascular necrosis of the capitate: report of six cases and review of the literature

Abstract: Avascular necrosis of the capitate is rare. Little is known about the aetiology, disease progression or optimal management. From 1992 to 2012 we treated six patients; four had a scaphocapitolunate arthrodesis and two had a four corner arthrodesis. The average follow up was 9 years (range 1-20). Three patients had good or excellent results, two fair and one poor, based on a visual analogue scale for pain and satisfaction and a Quick-DASH score. The Mayo wrist score was satisfactory in five cases and poor in one… Show more

“…Medical condition like gout, ligament instability, hematological disorders, systemic lupus erythematosus, and Gaucher's disease has a contributing effect, along with a history of local or systemic steroid use, smoking, and alcoholism. 3 Initial radiographs show sclerosis of the capitate; the collapse of the capitate head occurs early leading to decrease in the midcarpal joint space. The diagnosis is confirmed by MRI, which has a sensitivity of 95% and can show changes before radiographs.…”

“…The rare condition of osteonecrosis of the capitate bone typically manifests at the capitate head. 44,45 Like the lunate and scaphoid bone, the capitate receives its blood supply from nutrient vessels that have retrograde intraosseous courses after entering at the distal pole. Therefore, like the proximal segments of other carpalia, the capitate head is at particular risk of ischemia and subsequent necrosis.…”
