Some manifestations attributed to a primarily psychiatric disorder may result from an identified or unidentified neurological lesion; in other words, psychiatric symptoms are often a common determinant for both neurological pathologies and psychopathological disorders. The objective of this work was to study the radio-clinical profile of psychiatric manifestations attributable to a neurological process. We therefore carried out a retrospective cross-sectional study with a descriptive aim. The survey took place from December 2018 to March 2019 at the Abidjan Addictology and Mental Hygiene Service (SAHM) of the National Institute of Public Health (INSP). The period studied was 08 years (2012-2019). Thus, 47169 patient files were identified over this period (2012 to 2019). A total of 125 patient files meeting the inclusion criteria were selected from an exhaustive sampling. It should be noted that the use of specific biomarkers was not possible in our context. There was a modest female predominance of 52.80%, a mean age of 65 years. Clinical characteristics revealed a history of hypertension (23.2%). Call signs were amnesia (80.5%), incoherent speech (45.5%) and the delirium-hallucination dyad (40.9%). A dementia syndrome was evoked in a proportion of (61.6%); it required neuroimaging. In 96.8% of the cases, the brain scan revealed cortico-subcortical atrophy lesions (66.4%), but MRI could only be performed in (3.2%). Degenerative, vascular or mixed etiologies were evoked in 34.4%, 15.2% and 30.4% respectively. The risk of excessive psychiatry and misdiagnosis of behavioral disorders reveals the interest of a more elaborate neuropsychiatric evaluation and the use of neuroimaging.

> Les troubles de mémoire dans la maladie d'Alzheimer touchent très tôt la remémoration consciente, souvenir d'un événement et de son contexte. Pour ce qui est de la familiarité, reconnaissance sans souvenir du contexte, les études vont dans le sens tantôt d'une altération précoce, tantôt d'une préservation de la familiarité dans les stades légers et modérés de la maladie d'Alzheimer. Ce manque de consensus peut, en partie, être expliqué par des différences méthodologiques entre les études et révèle le caractère complexe de cette forme de mémoire qui serait sous-tendue par une multitude de mécanismes pouvant être sélectivement altérés ou préservés au cours de la maladie. < plusieurs fonctions cognitives, sans altération du niveau d'efficience cognitive globale et en l'absence de perturbation significative des activités de la vie quotidienne. Selon R.C. Petersen [1], différents sous-types de trouble cognitif léger peuvent être identifiés en fonction du domaine cognitif altéré. D'une part, le trouble cognitif léger de type amnésique regroupe les patients présentant une altération de la mémoire épisodique et, d'autre part, le trouble cognitif léger de type non amnésique regroupe les patients présentant une alté-ration cognitive autre que mnésique. Ces deux sous-types peuvent encore être subdivisés en fonction du nombre de domaines cognitifs altérés (un ou plusieurs). On sait aujourd'hui que ces différents sous-types de trouble cognitif léger n'évoluent pas vers les mêmes entités démentielles. Background The Integrated Care for Older People (ICOPE) approach was developed by the World Health Organization (WHO) aiming to shift the traditional focus of care based on diseases to a function- and person-centered approach, focused on maintaining and monitoring intrinsic capacity (IC). This study aimed to investigate the ability of the ICOPE screening tool to identify older people with clinically meaningful impairments in IC domains. Methods This cross-sectional analysis included 603 older adults, participants (mean age 74.7 [SD 8.8] years, women 59.0%) of the INSPIRE Translational (INSPIRE-T) cohort. Responses at screening were compared to results of the subsequent in-depth assessment (i.e., Mini Mental State Examination, Mini Nutritional Assessment, Short Physical Performance Battery, Patient Health Questionnaire-9, and clinical investigation of vision problems) to determine its predictive capacity for impairments at the IC domains (i.e., cognition, psychological, sensory (vision), vitality, and locomotion). Results The ICOPE screening items provided very high sensitivity for identifying abnormality in vision (97.2%), and varied from 42.0% to 69.6% for the other domains. High specificity (>70%) was observed for all the IC domains, except for vision (2.7%). Conclusions The ICOPE screening tool can be a useful instrument enabling the identification of older people with impairments in IC domains, but studies with different populations are needed. It should be considered as a low-cost and simple screening tool in clinical care.
