Many developing countries have highly unequal health systems across their regions. The pandemic of COVID-19 brought an additional challenge, as hospital structures equipped with doctors, intensive care units and respirators are not available to a sufficient extent in all regions. Using Data Envelopment Analysis, we create a COVID Index to verify whether the hospital structures in 543 Brazilian microregions are adequate to deal with COVID-19 and to verify whether public policies were implemented in the right direction. The results indicate that hospital structures in the poorest microregions were the most vulnerable, although the peak of COVID-19 occurred in the richest microregions (Sao Paulo). The Southeast states could relocate hospital resources or even patients between their regions. The relocation was not possible in many states in the Northeast, as the health system poorly assisted the interior of these states. These findings reveal that the heterogeneity of microregions’ hospital structures follows the patterns of socioeconomic inequalities. We conclude that it is easier for the wealthier regions to reallocate hospital resources internally than for the poorest regions. By using the COVID Index, policymakers and hospital managers have straightforward information to decide which regions must receive new investments and reallocate underutilized resources.

ResumoNeste artigo propõe-se analisar comparativamente o desempenho de sistemas de produção modais de pecuária extensiva de corte na fase de cria em 21 municípios brasileiros. A cria é a fase da atividade que sustenta toda a cadeia de produção da pecuária de corte. É usado o modelo de análise envoltória de dados (DEA) com retornos variáveis de escala. O objetivo é medir o desempenho da decisão do produtor em relação à composição do sistema produtivo. As variáveis usadas são número de reprodutores, quantidade de bezerros e vacas de descarte. Dos 21 sistemas avaliados, quatro foram DEA BCC eficientes e a maioria dos sistemas avaliados opera na região de retornos crescentes de escala. Palavras-chaveEficiência. DEA. Pecuária. The purpose of this study is to investigate the issue of efficiency in the Brazilian motor carrier industry using both DEA (Data Envelopment Analysis) and SFA (Stochastic Frontier Analysis) in a complimentary fashion. The study is based on secondary data collected from Transporte Moderno/Maiores e Melhores, a specialized magazine that annually reports statistics on the largest Brazilian trucking companies. Results corroborate not only that increasing returns to scale prevail within this industry, but also provide support for a moderate impact of economies of scope on efficiency levels. Implications in terms of mergers and acquisitions and the impact of cargo diversity and the geographical scope of the operation on virtual efficiency levels are also addressed.
