Em novembro de 2015 a barragem de rejeitos de mineração do Fundão, localizada no município de Mariana, estado de Minas Gerais, Brasil, entrou em colapso, despejando grande parte de seu conteúdo de cerca de 60 milhões de metros cúbicos, constituído por rejeitos de mineração de ferro, na bacia hidrográfica do Rio Doce. A onda de rejeitos percorreu um caminho de mais de 650 km, até chegar ao Oceano Atlântico, provocando o maior desastre ambiental da história do Brasil. A investigação das causas do desastre tem geralmente se restringido à identificação das causas de natureza técnica relacionadas mais à Engenharia Geotécnica, à Geologia e à Sismologia, buscando descrever e analisar os fenômenos que culminaram com o colapso da estrutura. Este trabalho teve por objetivo identificar os fatores que teriam contribuído para a ocorrência do fenômeno de liquefação, apontado como a causa imediata do colapso. Foi utilizada a técnica Análise de Constelação, mediante a realização de entrevistas com atores envolvidos, especialistas e peritos que trabalharam no caso. Os resultados mostraram que o rompimento da barragem foi decorrente, direta ou indiretamente, de ações e/ou omissões de diversos atores envolvidos no processo, que incluem projetistas, consultores, responsáveis pela construção e operação da barragem, gestores da empresa mineradora, órgãos responsáveis pela fiscalização da atividade minerária, entre outros.Palavras-chave: Barragem de rejeitos; Barragem do Fundão; Análise de Constelação Investigation on Causes of the Failure of Mining Tailings Dams: Case Study of the Fundão Dam, in Mariana, Minas Gerais, Brazil, Using the Constellation Analysis Technique A B S T R A C In November 2015, the Fundão tailings dam, located in the municipality of Mariana, State of Minas Gerais, Brazil, collapsed, spilling much of its content of about 60 million cubic meters, consisting of iron ore tailings, in the Rio Doce watershed. The tailings wave traveled a path of more than 650 km, until reaching the Atlantic Ocean, causing the biggest environmental disaster in the history of Brazil. The investigation of the causes of the disaster has generally been restricted to the identification of technical causes, related more to Geotechnical Engineering, Geology and Seismology, seeking to describe and analyze the phenomena that culminated in the collapse of the structure. The present work aims to identify, using the Constellation Analysis technique, through interviews with the actors involved, specialists and experts who worked on the case, the factors that would have contributed to the occurrence of the liquefaction phenomenon, identified as the immediate cause of the collapse. The results showed that the dam failure resulted, directly or indirectly, from actions and/or omissions of several actors involved in the process, which include designers, consultants, responsible for the construction and operation of the dam, managers of the mining company, bodies responsible for the supervision of mining activity, among others.Keywords: Tailings dam; Fundão Dam; Constellation Analysis
