Aim: The main aim of this study is to demonstrate how our melanoma disease model (MDM) can be used for cost-effectiveness analyses (CEAs) in the melanoma detection field. In particular, we used the data of two cohorts of Belgian melanoma patients to investigate the cost-effectiveness of dermoscopy. Methods: A MDM, previously constructed to calculate the melanoma burden, was slightly modified to be suitable for CEAs. Two cohorts of patients entered into the model to calculate morbidity, mortality and costs. These cohorts were constituted by melanoma patients diagnosed by dermatologists adequately, or not adequately, trained in dermoscopy. Effectiveness and costs were calculated for each cohort and compared. Effectiveness was expressed in (2016) 38e45 quality-adjusted life years (QALYs), a composite measure depending on melanoma-related morbidity and mortality. Costs included costs of treatment and follow-up as well as costs of detection in non-melanoma patients and costs of excision and pathology of benign lesions excised to rule out melanoma. Results: The result of our analysis concluded that melanoma diagnosis by dermatologists adequately trained in dermoscopy resulted in both a gain of QALYs (less morbidity and/or mortality) and a reduction in costs. Conclusion: This study demonstrates how our MDM can be used in CEAs in the melanoma detection field. The model and the methodology suggested in this paper were applied to two cohorts of Belgian melanoma patients. Their analysis concluded that adequate dermoscopy training is cost-effective. The results should be confirmed by a large-scale randomised study. ª

Early diagnosis of skin cancer, particularly melanoma, leads to improved morbidity and mortality. While nurses have been leaders in skin cancer awareness and education for decades, the nursing community can take a more active role in the fight against skin cancer. In order to assume this role, nurses must be familiar with diagnostic aids that help in the early recognition of skin cancer. Dermatology nurses facilitate care in the interdisciplinary team by focusing on patient centered outcomes. Nursing roles and responsibilities in the interdisciplinary team are vital to clinic pre-screening, improving public awareness, disseminating patient education, providing guidance regarding sun avoidance and protection, and providing education on the fundamentals of skin self-examinations and total body skin examinations. Nursing skin assessment requires knowledge of skin lesion morphology and biology, and pattern recognition. As the sensitivity and specificity of naked eye examinations are suboptimal, dermoscopy provides a method for improving and streamlining skin lesion triage and assessment. In this review, we discuss a multi-prong approach to the diagnosis of melanoma, including the ABCDE mnemonic, the "ugly duckling" concept, and some newer technologies (e.g., dermoscopy and total body photography) that aid in the early detection of skin cancers. Familiarity with these detection aids can provide nurses with a basic framework to aid in diagnosing skin cancer.

GirişGünümüzde dermoskopi, başta melanom erken tanısı olmak üzere, infeksiyöz ve inflamatuvar dermatozlar da dahil olacak şekilde dermatolojinin hemen her alanında tanı ve tedavi takibi açısından büyük önem taşımaktadır. Eğer bir dermatolog kendini bu konuda yeterince eğittiyse, el dermoskobunu rutin hasta muayenesinin temel bir parçası haline getirebilir. Bu makalenin konusu olan "dermoskopide hasta yönetimi", genel dermatolojide kullanımından çok, dermoskopinin temel amacı olan melanomun erken tanınması ile ilgili yapılacakların iyi organizasyon ve uygulamasını ifade etmektedir.Dermoskopide hasta yönetimi anlayışı geliştirmemiz, birçok nedenle günümüzde zorunlu hale gelmektedir. Hasta sayısının çokluğuna karşın iyi yetişmiş insan gücünün sınırlı olması, önemli bir zaman sorunu oluşturmakta ve yapılan işin maliyet etkin (cost-effective) olup olmadığı sorusunu gündeme getirmektedir. Maliyet etkin ifadesi, sadece parasal açıdan değil, zaman ve enerji açısından da yapılan işe değer olma anlamını taşımalıdır.
