Autotransplantation of fragmented ovarian cortical tissue: a laparoscopic demonstration

Abstract: Objective: To describe and demonstrate a simple and secure procedure for laparoscopic autotransplantation of fragmented ovarian cortical tissue in women with diminished ovarian reserve as part of in vitro activation (IVA) of ovarian follicles. Design:Step-by-step video explanation of the surgical procedure with still pictures and surgical video clips to demonstrate the detailed technique. Setting: Fertility clinic and obstetrics and gynecology department at a university hospital. Patient(s): Women with idiopat… Show more

“…Firstly, they conducted a similar drug-free IVA in 20 POI patients. There were no recorded complications and 12 patients could achieve pregnancies ( 74 , 75 ). There was no significant difference in the number of mature follicles and the AMH levels between the treated group and the control one during 10 weeks of observation ( 75 ).…”

“…In short, ovarian cortex fragments from POI patients were vitrified, warmed, fragmented into small pieces (approximately 100 cubes of 1-2 mm 2 ) and incubated with PTENinhibiting and PI3K-Akt-stimulating drugs, respectively, for 2 days before laparoscopic auto-transplantation beneath the serosa of the fallopian tubes. To this day, only a small number of pregnancies and births resulting from this method have been reported in the peer-reviewed literature or in the form of an abstract presentation (Fabregues et al, 2018;Lunding et al, 2018;Lee and Chang, 2019).…”
