Autonomic Dysfunction in the Synucleinopathies

Abstract: Autonomic dysfunction is a characteristic feature in the synucleinopathies. Differences in cellular deposition and neuronal populations affected by α-synuclein aggregation influence the manifestations and severity of autonomic failure in the different synucleinopathy disorders. The Lewy body disorders (Parkinson's disease, dementia with Lewy bodies, and pure autonomic failure) have predominantly peripheral involvement, whereas multiple system atrophy chiefly manifests as central autonomic failure. Clinical and… Show more

“…Cardiovascular impairment, gastrointestinal dysfunction, and urinary disturbance are some of the key features of autonomic dysfunction in PD. 1 Orthostatic hypotension (OH), which is defined as a sustained drop in systolic blood pressure >20 mm Hg and/or a drop in diastolic blood pressure >10 mm Hg within 3 minutes from supine to standing, causes considerable limitations and impairment in quality of life. 2 OH may be neurogenic or non-neurogenic.…”

“…Patients with Parkinson’s disease may also have difficulty maintaining core temperature and can develop hypothermia, due to median preoptic lesions. [ 21 ] However, a description of the most characteristic and conspicuous signs and symptoms of Parkinson’s disease (e.g., bradykinesia, rest tremor, or rigidity) and DLB (e.g., recurrent visual hallucinations or REM sleep behavior disorder) is missing from the biblical text, thus seriously weakening the case for the diagnostic hypothesis of synucleinopathy.…”

“…Unlike PD, DLB, and MSA, Pure autonomic failure (PAF) is a rare sporadic neurodegenerative disease induced by dysfunction of autonomic nervous system (Thaisetthawatkul, 2016). It is slowly progressive disease and caused by abnormal accumulation of α-syn (Thaisetthawatkul, 2016;Coon et al, 2019;Coon, 2020). PAF patients showed orthostatic hypotension, syncope, falls, sexual and bladder dysfunction, and constipation with no motor dysfunction (Stubendorff et al, 2012;Allan, 2019).…”
