The recent outbreak of respiratory syndrome-coronavirus-2 (SARS-CoV-2), which causes coronavirus disease (COVID-19), has led to the widespread use of therapeutics, including dexamethasone (DEXA). DEXA, a synthetic glucocorticoid, is among the widely administered drugs used to treat hospitalized COVID-19 patients. The global COVID-19 surge in infections, consequent increasing hospitalizations, and other DEXA applications have raised concerns on eminent adverse ecological implications to aquatic ecosystems. Here, we aim to summarize published studies on DEXA occurrence, fate, and effects on organisms in natural and engineered systems as, pre-COVID, the drug has been identified as an emerging environmental contaminant. The results demonstrated a significant reduction of DEXA in wastewater treatment plants, with a small portion, including its transformation products (TPs), being released into downstream waters. Fish and crustaceans are the most susceptible species to DEXA exposure in the parts-per-billion range, suggesting potential deleterious ecological effects. However, there are data deficits on the implications of DEXA to marine and estuarine systems and wildlife. To improve DEXA management, toxicological outcomes of DEXA and formed TPs should entail long-term studies from whole organisms to molecular effects in actual environmental matrices and at realistic exposure concentrations. This can aid in striking a fine balance of saving human lives and protecting ecological integrity.

A fully automated integrated analytical platform that included continuous water sampling, sample preparation (extraction), and analysis for the determination of nine glucocorticoids was developed. A peristaltic pump was employed to deliver the water sample automatically. The subsequent sample preparation and analytical processes were also fully automated with the use of a commercial autosampler unit coupled to an ultrahigh performance liquid chromatography–tandem mass spectrometric system. The sample preparation step involved membrane bag liquid-phase microextraction in which a synergistic mixture of n-octanol enhanced with surfactant sodium dodecyl sulfate (SDS) (0.10% in n-octanol) was applied as the extractant phase. Addition of SDS helped to improve the extraction, increasing enrichment factors (EFs) by severalfold compared with the use of n-octanol only as extractant solvent. Various parameters to improve extraction efficiency were evaluated. They included percentage of surfactant used, sample pH, agitation rate, extraction duration, salting out effect, extraction temperature, and flow rate for sample introduction. Under the most favorable conditions, enrichment factors of between 32 and 189 were attained with relative standard deviations of <8%. The limits of detection for the glucocorticoids were between 0.03 and 0.17 ng/mL, whereas the limits of quantitation were between 0.09 and 0.52 ng/mL, indicating the good sensitivity of the method. The method was successfully applied to determine glucocorticoids in industrial canal water. This work also provided some insights on the potential of on-site sampling and real-time monitoring of contaminants in environmental waters.

Recent years have seen the increased utilization of ionic liquids (ILs) in the development and optimization of analytical methods. Their unique and eco-friendly properties and the ability to modify their structure allows them to be useful both at the sample preparation stage and at the separation stage of the analytes. The use of ILs for the analysis of pharmaceuticals seems particularly interesting because of their systematic delivery to the environment. Nowadays, they are commonly detected in many countries at very low concentration levels. However, due to their specific physiological activity, pharmaceuticals are responsible for bioaccumulation and toxic effects in aquatic and terrestrial ecosystems as well as possibly upsetting the body’s equilibrium, leading to the dangerous phenomenon of drug resistance. This review will provide a comprehensive summary of the use of ILs in various sample preparation procedures and separation methods for the determination of pharmaceuticals in environmental and biological matrices based on liquid-based chromatography (LC, SFC, TLC), gas chromatography (GC) and electromigration techniques (e.g., capillary electrophoresis (CE)). Moreover, the advantages and disadvantages of ILs, which can appear during extraction and separation, will be presented and attention will be given to the criteria to be followed during the selection of ILs for specific applications.
